This case is of a 47 year old African American female diagnosed with catatonic schizophrenia and paranoid schizophrenia which has deteriorated over the past two weeks. The patient recently attacked her father, has been mute and has also been throwing herself onto the floor expressing disorganized thoughts, poorly taking care of self and insomnia. The patient was first admitted for psychiatric care at the age of 20 and has a past history of DVT of lower extremities, hypertension, obesity and anemia. No signs of intoxication or drug withdrawal were observed. Patient had been on Haldol decanoate 300 mg IM. After admission in ward patient was continued on Haldol decanoate, Depakote and Lorazepam. Patient responded well to treatment and a certain level of independence wasachieved

Background: Catatonia is a syndrome comprising psychomotor, behavioral, and autonomous symptoms which may occur in the context of severe schizophrenic, affective, and other mental disorders or medical conditions. Treatment options include high dose benzodiazepines (lorazepam) and electroconvulsive therapy (ECT) with some evidence for the effectiveness of glutamate antagonists. However, due to a lack of randomized controlled studies in this severely ill population, evidence base is weak. Methods: On occasion of the case of a patient with treatment resistant catatonia in schizoaffective disorder, we developed the hypothesis of vagus nerve stimulation (VNS) being a potential therapy for treatment resistant catatonia. Results: Based on a selective literature search, we found a remarkable overlap of the pathophysiology of catatonia on the one hand and the putative mechanisms of action of VNS on the other hand in several domains: functional brain imaging, involved neurotransmitter systems, clinical, and theoretical. We thus decided to use VNS as a single subject clinical trial. During the 1-year-follow-up, we observed a fluctuating, but ultimately marked improvement of both catatonic symptoms and general psychopathology. Conclusions: We assume there is a sufficient hypothetical corroboration for the potential effectiveness of VNS as a long-term treatment in predominantly catatonic syndromes. This hypothesis could be tested in proof-of-concept clinical trials.

РезюмеОбоснование: связь кататонических расстройств с аффективной патологией отмечается в работах, относящихся еще к донозологическому этапу исследований в психиатрии. Клиническим исследованиям этой проблемы посвящены многочисленные публикации минувшего века и последних десятилетий наступившего. На настоящем этапе развития психиатрии кататония рассматривается в качестве транснозологического образования, манифестирующего в рамках различных, в том числе аффективных заболеваний. Цель: теоретический анализ развития представлений об аффективно-кататонических расстройствах в период XIX-XXI вв. и построения концептуальной модели аффективно-кататонических состояний, формирующихся в клиническом пространстве шизофрении и расстройств шизофренического спектра. Заключение: аффективно-кататонические расстройства представляют собой континуум психопатологически дифференцированных фазно протекающих состояний. На одном из полюсов континуума кататонические расстройства, манифестирующие в структуре аффективной патологии и реализующиеся на уровне рекуррентных депрессий/гипоманий, на другом -аффективно-кататонические состояния более тяжелого психопатологического регистра, выступающие в клиническом пространстве биполярного аффективного расстройства и шизоаффективных психозов. Соучастие двигательных расстройств в формировании клинической картины аффективно-кататонических приступов и фаз, а также модус взаимодействия кататонических расстройств с аффективными варьируются в широких пределах. В одних случаях двигательные симптомокомплексы вытесняют аффективную симптоматику, на базе которой они сформировались, и целиком определяют клиническую картину, в других -лишь модифицируют проявления аффективных расстройств, не видоизменяя их синдромальной структуры.
