ZusammenfassungDie neuen Erkenntnisse der Pathophysiolgie des Trockenen Auges erkennen das Zusammenspiel von Tränen, Augenoberfläche und Lidoberfläche als eine funktionelle Einheit an. Der Begriff der Benetzungsfähigkeit der Tränen in Abhängigkeit der mikrotektonischen Anatomie der Augenoberfläche relativiert die Anforderungen an Träne und Tränenersatzmittel. Das Model der Attrition, welches die Effekte der friktionsneutralisierenden Kapazität des Tränenfilms, der Reibung und die Bedeutung der Mechanotransduktionskapazität des Epithels zusammenfasst, wird eingeführt und dessen pathophysiologische Bedeutung erläutert. Attrition und Benetzung bestimmen zusammen grundlegende pathophysiologische Vorgänge in der Augenoberfläche wie Aktivierung von Nerven (subjektive Beschwerden) sowie Entzündung und beeinflussen damit die Dynamik der Pathophysiologie, und den Übergang von vorübergehenden Beschwerden zu einer manifesten Erkrankung des Trockenen Auges. Die Betrachtung der Osmolarität als numerischer statischer Grenzwert zur alleinigen Diagnose des Trockenen Auges ist klinisch nicht haltbar. Das neue, dynamische Model der Osmokinetik, zeigt dagegen eine Alternative auf, in der die Tageschwankungen und die Beachtung des durchschnittlichen Osmolaritätsniveaus gröβere Bedeutung gewinnen und damit der eigentlichen pathophysiologischen Bedeutung der Osmolarität gerechter wird.

Cataract is a disease opacifying the crystalline, leading to a blurred vision and ultimately to blindness. With an aging population, the incidence of cataract is increasing, as well as the number of treatments. The solution available is its complete removal, followed by an implant of an intraocular lens (IOL). Although the post-operative complications on cataract surgeries have been decreasing in general, the bag-IOL complex dislocation is still an issue, probably being the most serious complication under this procedure. In this work, an axisymmetric Finite Element (FE) modelling strategy of the crystalline complex during the process of accommodation under cataract surgery is proposed. The goal was to understand the influence of biomechanical alterations promoted by the IOL on bag-IOL dislocation after surgery. An increase of force and stress in the zonules was verified in the pseudophakic eye compared to the complete eye, which could explain why zonules break years after surgery, leading to the bag-IOL dislocation. The axisymmetric FE model proposed in this work is innovative in this field, which still lacks detailed research, and can be an important complement for the clinical and biomechanical work on the crystalline complex.

Background: After strabismus surgery, the local swelling of conjunctival wound will affect the uniform distribution of tears on the ocular surface, and the inflammatory reaction will affect the stability of tear film, which will easily lead to iatrogenic dry eye. This study aimed to investigate the effect of vitamin A palmitate (VAP) eye gel application in dry eye and the advantages of Watson's theory of caring.Methods: Two hundred and forty children with dry eye after strabismus surgery treated in our hospital from September 2018 to September 2020 were selected as the study subjects, and were randomly divided into control group and observation group according to the allocation ratio of 1:1. Watson's theory of care nursing was applied in the observation group, and VAP eye gel was additionally dropped into the eye. Mouse conjunctival goblet cells (GCs) were used to detect the effect of VAP on the growth of GCs. Treatment compliance, improvement of dry eye symptoms [Schirmer I test (SIT), tear film break-up time (BUT), and fluorescent staining (FL) score], inflammatory factor levels in tears, clinical efficacy, and parents' satisfaction were compared.Results: It was found that VAP eye gel could better promote the proliferation of GCs. After nursing and clinical treatment, the dry eye symptoms were improved in all included children, and improvements in the SIT, BUT, and FL scores were more obvious in the observation group. Watson's theory of care nursing could effectively improve the children's treatment compliance and parents' satisfaction.Conclusions: As a result, the application of VAP eye gel and Watson's theory of care nursing could effectively reduce the occurrence of dry eye after strabismus surgery, and were of great importance for improving the relationship between nurses and patients and building a harmonious hospital.
