Case Series Outcomes at 1 Week Following Verteporfin Photodynamic Therapy

Abstract: The absence of fluorescein leakage from classic CNV at 1 week in all cases was consistent with the published outcome of the clinical Phase I and II PDT trials. Further, vision loss 1 week after PDT for predominantly classic CNV was very uncommon. Therefore 1-week post-PDT angiography is unnecessary for predominantly classic CNV in patients with AMD.

“…Extensive closure of the normal choroid after PDT can be related (and may provide a possible explanation) to reports indicating decreased visual acuity at week 1 after PDT in some patients. 34 It is generally believed that choriocapillaris hypoperfusion recovers within 3 months of PDT. 35,36 However, in the present study, we observed areas of hypoperfusion even at 9 months.…”

“…Changes in subretinal fluid have been angiographically described after treatment as accumulation of fluid immediately after PDT of which inflammation might be responsible, followed by a reduction of subretinal fluid in the weeks afterwards, and finally a resolution or inactivation of the CNV [12].…”

“…The OCT has been previously used to morphologically study eyes with AMD and proved its usefulness in defining occult and classic CNV, the presence of subretinal and intraretinal fluid, serous, hemorrhagic and fibrovascular detachments of the retinal pigment epithelium (RPE), and disciform scarring [4,12].…”
