Case report: Transcatheter aortic valve replacement in a large bicuspid anatomy using the XL-Myval 32 mm

Abstract: Transcatheter aortic valve replacement (TAVR) is a recommended intervention for selected population with severe aortic stenosis (AS). Bicuspid aortic valve (BAV) anatomy has been categorized as an unfavorable anatomy for TAVR due to multiple considerations as exclusion from randomized trials in addition to the challenging and unpredictable anatomy. The anatomical constraints of BAV include the large anatomy of the annulus, sinus of Valsalva, and aorta (aortopathy), in addition to significant calcifications of … Show more

“…Ltd., India) is a balloon-expandable valve available in larger sizes, 30.5 and 32 mm, serving to treat patients with large annulus. 2 The available bigger sizes are encouraging clinicians to use this valve in aortic regurgitation with enlarged annulus. It can protect against valve embolisation and residual PVL in non-calci ed aortic valves with dilated annulus and hence can be a good option for treatment of pure AR in high risk for SAVR patients.…”

“…Ltd), offers a solution, particularly bene cial when larger valve sizes are required. 2 Additionally, in instances where peripheral access is prohibitive, a trans-apical approach(through left anterior thoracotomy) or a direct aortic approach (performed through upper median sternotomy or anterior right mini-thoracotomy) become necessary.…”
