Case Report-Inguinoscrotal ureteral hernia diagnosed on micturating cystourethrography

Abstract: The presence of a ureter within an inguinal hernia is an extremely rare entity, usually discovered incidentally during herniorrhaphy and may pose a surgical risk. Early preoperative diagnosis is crucial to guide proper surgical approach and to preserve renal function.

“…The presence of prostatic enlargement with dilatation of proximal urinary tract as seen in our patient was previously reported by Kate et al [8]. Preoperative diagnosis of groin hernia containing the ureter or bladder can be made with a micturating cystogram [9] or an intravenous urogram. This could help avoid ureteric injury during surgery by delineating its path.…”

“…Resection of the herniated segment of the ureter is recommended in case of injury, redundancy, or other pathologic conditions [5,9,15]. Burgu et al [6] and Sripathi et al [11] have performed an end ureterostomy of the extremely dilated ureter probably with the intention of later reimplantation. The options for repairing long ureteral defects include psoas hitch, Boari flap, transureteroureterostomy, autotransplantation, and ileal or appendiceal interposition.…”

“…In females, the ectopic ureter usually opens below the level of the urinary sphincter causing incontinence, however a supra-sphincteric insertion in males preserves continence. Inguino-scrotal herniation of the urinary bladder within a "sliding hernia" (in which the organ involved forms part of the wall of the hernia sac) is not uncommon, however ureteric involvement (native or transplanted) is rare [1,2]. Eighty per cent are paraperitoneal, where the hernia has peritoneum antero-medially in addition to the ureter (and other organs) adherent to the postero-lateral peritoneum as part of the sac [3].…”
