Case report: elective removal of a large mesenteric cyst—our approach

Abstract: A mesenteric cyst is rare intra-abdominal pathology, with little literature to guide us on how to diagnose and manage it. We report the incident of a 57-year-old female who had an incidental finding of a sigmoid mesenteric cyst whilst undergoing an operation under the care of the Gynaecologists. A computed tomography scan and a flexible sigmoidoscopy followed to help diagnose the lesion as a cyst. A month later the 10 × 15 cm2 cyst was excised laparoscopically with no complications.

“…The clinical examination findings are determined by the size of the cyst. The diagnosis is based on a thorough clinical examination as well as radiological modalities such as ultrasonography and computed tomography (CT) scans [ 2 , 3 , 5 ]. Our patient presented with an unusual combination of a mesentery cyst and acute appendicitis; appendectomy and enucleation of the cyst were done via laparotomy; this co-occurrence was found only in two children during the literature review.…”

“…При этом следует учитывать, что наиболее эффективен вариант полного удаления кисты в пределах здоровых тканей единым блоком, без вскрытия ее просвета, что минимизирует риск возникновения рецидива. Кроме того, в случае осложненного течения может возникнуть необходимость резекции прилегающих к кисте структур [15]. На сегодняшний день остается актуальным вопрос, какой операционный доступ является наиболее подходящим для данной нозологической категории.…”

“…Mesenteric cysts are rare intra-abdominal benign tumors (1 in 100,000 cases in adults) with various clinical presentations. [1,2] They commonly originate from the small bowel mesentery, although a proportion has been found to originate from the mesocolon, and retroperitoneum. [1,3] The formation of mesenteric cysts depends on the histologic origin, where they could be classified into cysts of lymphatic origin, cysts of mesothelial origin, cysts of enteric origin, cysts of urogenital origin, dermoid cysts and pseudocysts.…”
