Cartilage quality in rheumatoid arthritis: comparison of T2* mapping, native T1 mapping, dGEMRIC, ΔR1 and value of pre-contrast imaging

Abstract: The data support the use of dGEMRIC for the assessment of MCP joint cartilage quality in RA. T2* and native T1 mapping are of low diagnostic value. Pre-contrast T1 mapping for the calculation of ΔR1 does not increase the diagnostic value of dGEMRIC.

“…So far, a few studies have applied dGEMRIC and T2 mapping for the assessment of cartilage changes in healthy control subjects and patients with RA 7 11 15 16. These data, conducted in a small number of subjects, have shown that both techniques are feasible to measure the biochemical properties of cartilage in the finger joints.…”

“…Several specialized MRI techniques are available for the detection and quantification of early cartilage damage such as delayed Gadolinium-Enhanced MRI of the Cartilage (dGEMRIC), T2 mapping, T1-sodium MRI, and cartilage volume/thickness measurements. 32,33 With advanced cartilage disease in inflammatory arthritis, chondral loss is seen throughout the articular surface in contradistinction to OA where cartilage damage mainly affects the weightbearing surfaces. In addition, cartilage loss in inflammatory conditions often involves the bare areas of the joint that are located adjacent to the joint capsule.…”

“…Niedrige dGEMRIC Signale korrelieren mit weniger GAG Gehalt des Knorpels [26]. Es konnte gezeigt werden, dass dies auch im Knorpel der MCP Gelenke gesunder dargestellt werden kann [27]. So korreliert das Ausmaß der entzündlichen Veränderungen des RAMRIS (für Synovialitis und Knochenmarködem) deutlich besser mit der Abnahme der Knorpelqualität als der Erosionsscore [28].…”
