Aim. Increased cardiovascular morbidity and mortality have been observed in ankylosing spondylitis because of accelerated atherosclerosis. We measured carotid intima media thickness (CIMT) as a surrogate marker of atherosclerosis in this study. Methods. In this study 37 cases of AS and the same number of matched individuals were recruited. CIMT measurements were done using B-mode ultrasound. Disease activity was assessed using Bath ankylosing spondylitis disease activity index (BASDAI), Bath ankylosing spondylitis functional index (BASFI), and Bath ankylosing spondylitis metrological index (BASMI) scores and C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) levels. Results. Mean age of the study groups was 29.43 ± 9.00 years. Average disease duration was 65.62 ± 54.92 months. Twenty-eight (75.68%) of cases were HLA B-27 positive. A significantly increased CIMT was observed in cases as compared to control group (0.62 ± 0.12 versus 0.54 ± 0.04; P < 0.001). CIMT in the cases group positively correlated with age (r = 0.357; P < 0.05), duration of disease (r = 0.549; P < 0.01), and BASMI (r = 0.337; P < 0.05) and negatively correlated with ESR (r = −0.295; P < 0.05). Conclusions. Patients of AS had a higher CIMT than those of the control group. CIMT correlated with disease chronicity.

Karotis intima-media kalınlığı (KİMK), Ankilozan spondilit (AS) ve 25 (OH) D3 seviyesi arasındaki ilişkiyi açıklamak. Gereç ve Yöntem: Bu analizde, AS tanılı altmış üç erkek hasta ve 47 erkek kontrol değerlendirildi. AS hastalık aktivitesin değerlendirmede, Bath ankilozan spondilit hastalık aktivite indeksi (BASDAI), Bath ankilozan metrolojik indeksi (BASMI) ve Bath ankilozan spondilit fonksiyonel indeks (BASFI) skorları kullanıldı. Serum lipid düzeyleri ölçüldü. KİMK ultrasonografi ile ölçüldü. 25(OH)D3 vitamin düzeyleri ≤ 15 ng/ml Vitamin D eksikliği olarak tanımlandı. Bulgular: Hastalar ve sağlıklı kontrollerin yaş ortalaması 39,93 + 9,7 ve 40,55 + 9,7 yıl idi. Serum 25 (OH) D3 seviyesi kontrol grubu ile karşılaştırıldığında, AS'li hastalarda anlamlı derecede düşük bulundu (p = 0.034). KİMK ölçümleri kontrol grubuna göre AS grubunda yüksek bulundu, ancak istatistiksel olarak anlamlı değildi (p = 0.081). KİMK ölçümleri ile; yaş (r = 0.655, p <0.001), VKİ (r = 0.270, p = 0.033), hastalık süresi (r = 0.683, p <0.001), BASMİ (r = 0.409, p = 0.001), BASFI (r = 0,266, p = 0.035), total kolesterol düzeyleri (r = 0.290, p = 0.021) arasında pozitif korelasyon bulundu. High dansite lipoprotein düzeyleri; 25 (OH) D3>15 ng/ml olan hastalar ile karşılaştırıldığında 25 (OH) D3≤15 ng/ml hastalarda anlamlı olarak daha düşük olduğu tespit edildi (p = 0,001). Tartışma: Bu çalışmada, AS'li hastalardaki fonksiyonel ve mobilite limitasyonları ile KİMK arasındaki ilişki gösterilmiştir. Bunun yanında, D vitamini seviyesi high dansiteli lipoprotein düzeyleri ilişkilidir ve indirekt olarak aterosklerozu etkileyebilir.

Evaluation of Atrial Conduction Times, Epicardial Fat Thickness and Carotid Intima-Media Thickness in Patients With Ankylosing Spondylitis

Objectives: This study aims to determine the relationship between atrial electromechanical delay (EMD), carotid intima-media thickness (CIMT), and epicardial fat thickness (EFT) in ankylosing spondylitis (AS), which has a complicated inflammatory nature. Patients and methods:The study population included 42 consecutive patients with AS (28 males, 14 females; mean age 39.3±8.5 years; range 22 to 60 years) and 40 healthy subjects as controls (24 males, 16 females; mean age 37.2±8.7 years; range 22 to 60 years) (p>0.05). All patients underwent a standard tissue Doppler echocardiography to assess the left ventricular diastolic dysfunction, atrial EMD, CIMT, and EFT. All values were compared between the groups. Results: Interatrial (29.5±5.8 ms vs. 17.9±5.3 ms) left and right intraatrial EMD (18.2±4.6 ms and 11.7±3.5 ms vs. 11.9±3.2 ms and 7.1±3.2 ms, respectively) intervals were longer in AS patients than in healthy controls (all p<0.001). Left and right CIMT (0.50±0.11 mm and 0.44±0.06 mm vs. 0.51±0.11 mm and 0.43±0.04 mm, respectively) and EFT (0.73±0.15 cm and 0.63±0.07 cm) values were higher in AS patients than in healthy controls (all p<0.01). Conclusion: To our best knowledge, this is the first report evaluating the atrial EMD, CIMT, and EFT values together in AS patients. As indicators of cardiovascular involvement, all parameters were higher in AS patients.
