Carotid Intima Media Thickness as a Measure of Cardiovascular Disease Burden in Nigerian Africans with Hypertension and Diabetes Mellitus

Abstract: As part of a larger study of cardiovascular risk factors in nonhypertensive type 2 diabetes patients, we subjected a cohort of diabetics to B mode ultrasonography of the carotid artery to measure the intima media thickness (IMT) and compared it with values in hypertensives and apparently normal controls matched reasonably for gender and age. All groups were comparable in terms of age and gender representation. The mean (SD) of carotid IMT right and left was 0.94 mm (0.12), 0.94 mm (0.16); 0.93 mm (0.21), 0.93 … Show more

“…Our study demonstrates that patients with diabetes have significantly higher CIMT compared to apparently healthy individuals. Although this finding conforms to reports from other parts of Nigeria and the world [12,18], measured CIMT among the diabetics in our study is much higher. Plausible explanation for this variation includes differences in sample sizes, duration of diabetes, differences in demographics, co morbidities, method of measuring CIMT and levels of glycaemic control.…”

“…Diabetes have been associated with increased CIMT, and this has been linked to increased cardiovascular events in Caucasians and Asians with type 2 diabetes mellitus (T2DM) [9][10][11]. Among the patients studied in Jos (central Nigeria), CIMT was found to be higher among diabetics than the non-diabetics [12].…”

“…Heterogeneity in the methods used by different studies at the site of measurement of carotid artery IMT could account for this variation. In this study, measurement of the CIMT in the longitudinal and the transverse planes was employed, and the average of these 2 planes was used as a single CIMT value for each side, similar to previous studies by Casella et al and Okeahialam et al The anterolateral and posterolateral planes, in relation to the sternocleidomastoid muscle, were used for the longitudinal measurement due to clarity and easy accessibility to the CCA, as supported by Casella et al…”
