Caries and background factors in Swedish 4-year-old children with special reference to immigrant status

Abstract: Although the proportion of children with caries declined between 2007-2012, this decline was limited to non-immigrant children. Since 1980 the distribution of dmfs remained unchanged among children with caries. More research on interventions for changing oral health behaviours is needed, specifically for immigrant children.

“…In addition, oral health behaviour, such as toothbrushing frequency and sugar consumption, might play a mediating role in the association between ethnicity and dental caries. Obviously, oral health behaviour is the main determinant for oral health, but it also appears to be associated with immigrant status [Stecksén-Blicks et al, 2014]. Our hypothesis is that ethnic disparities in caries prevalence can be mostly explained by SES and differences in oral health behaviour between ethnic groups.…”

“…Thus far, European studies did not distinguish between major, country-dependent, ethnic minority groups [Marthaler, 2004;Truin et al, 2007;Wigen and Wang, 2010;Fontana et al, 2011;Schuller et al, 2013;Stecksén-Blicks et al, 2014]. It is important to fill this knowledge gap, to be able to aim prevention programmes at the most vulnerable groups.…”

“…This was also true when caries frequency was compared. Our result was in accordance with previous studies from Sweden (Bankel, Eriksson et al 2006, Jacobsson, Koch et al 2011, Stecksén-Blicks, Hasslöf et al 2014) and other Scandinavian countries (Skeie, Riordan et al 2006, Christensen, Twetman et al 2010. The number of children with CL/P and immigrant background (n = 9) was too small and therefore suboptimal for looking at differences in caries prevalence/frequency and birthplace.…”

“…The reason for this seems to be a high consumption of sugary products and poor oral hygiene within these risk groups (Skeie, Riordan et al 2006, Stecksén-Blicks, Hasslöf et al 2014.…”

“…SES indices based on individuals are uncommon in dentistry [Havard et al, 2008]. Hitherto, mostly specific SES factors, such as immigrant status, have been studied in relation to children's dental health [Wendt et al, 1999;Julihn et al, 2010, Stecksén-Blicks et al, 2014. Only a few Swedish studies have investigated the influence of more than 1 SES factor on dental health, like education, income, and immigrant status [Hjern et al, 2001;Källestål and Wall, 2002], and the combined effect of multiple SES factors based on individuals has been less explored.…”
