Caregivers' Self-Reported Absence of Social Support Networks is Related to Treatment Abandonment in Children With Cancer

Abstract: We found a strong association between the absence of SSN and TxA that was independent of other covariates, including surrogate measures of wealth. Our findings highlight the imperative understanding of social ties and support surrounding children's families for planning strategies to prevent TxA.

“…Determinants of TxA in our study (socioeconomic factors, perceived prognosis of disease, concerns about toxicity, and preference for TCAM) are very similar to those worldwide, as established in a global online survey, a systematic review, and other reports . Strategies to reduce abandonment are being studied and increasingly reported . Most centers have adopted overlapping strategies that stem from an increased understanding of the deficiencies common to most LMIC settings.…”

“…Most centers have adopted overlapping strategies that stem from an increased understanding of the deficiencies common to most LMIC settings. These usually tend to focus on one or more aspects of support and include setting up of local foundations, enhanced psychosocial support, education of families, subsidized or free treatment, food, transport, and lodging for families, and development of adapted treatment protocols, all of which have been found to decrease rates of TxA . A few reports have also focused on surveillance of treatment adherence and prospective tracking of patients, coupled with timely intervention .…”

“…The low incidence rates observed in low-resource regions might in part have resulted from underdiagnosis, 13 but our analysis could not quantify its extent. Treatment abandonment, common in LMICs, 14 might also have led to under-registration if identified patients who refused treatment were not sufficiently well described (eg, missing dates or place of residence). Under-ascertainment of diagnosed cases might have resulted from administrative restrictions of access to medical files, political or social instability, competing needs, and inadequate political will, causing a dearth of resources, shortage or volatility of registry personnel, loss of perennial expertise, and missing or broken links with relevant data sources.…”
