Care Coordination Can Reduce Unmet Needs of Persons With Severe and Persistent Mental Illness

Abstract: Introduction: Persons with severe and persistent mental illness (SPMI) have multiple and complex needs, many of which are not health related. Mental health services are unable to address these needs without collaboration with other agencies.
