Context: Cardio vascular disease (CVD) is the leading cause of mortality and morbidity in diabetes mellitus (DM) contributing to 65% of all deaths with diabetic complications. The most important cause of CVD is atherosclerosis, and dyslipidemia acts as a marker of developing atherosclerosis. The derangement of lipid profile and atherogenic indices start in the prediabetic state, much before the development of DM. Detection of the deranged lipid profile and atherogenic indices in the prediabetic state can help devise the aggressive treatment strategy right from this stage, so as to arrest the development of CVD as a complication of diabetes. Aims: To compare the lipid profile and atherogenic indices of prediabetics with controls and diabetics. Settings and Design: The electronic medical records of 239 subjects were reviewed retrospectively. Materials and Methods: About 187 cases consisting of 137 diabetics and 50 prediabetics were evaluated for serum fasting blood sugar, post prandial blood sugar, HbA1c, total cholesterol (TC), triglycerides (TGs), high-density lipoprotein cholesterol (HDLc), and low-density lipoprotein cholesterol (LDLc). Atherogenic indices [TC/HDLc, LDLc/HDLc, (TC-HDLc)/HDLc, TG/HDLc] were also evaluated in the two groups. Rest 52 age- and sex-matched subjects were taken as controls. Statistical Analysis Used: The comparisons were evaluated using SPSS statistical package version 20. Results: TC, TG, LDLc, and the atherogenic indices were significantly increased in prediabetics as compared with controls. HDLc was significantly decreased in prediabetics. Conclusions: The altered lipid profile and atherogenic indices in prediabetics signify the increased susceptibility of prediabetics to CVD in the long run. Hence, we recommend screening of prediabetics for dyslipidemia to arrest the development of cardiovascular complications.

Most data on the burden of diabetes and prediabetes are from countries where local infrastructure can support reliable estimates of the burden of non-communicable diseases. Countries in the Middle East and Africa, together with Russia, have a total population of almost 2 billion, but have been relatively overlooked by authors in this field. We reviewed the prevalence and drivers of prediabetes and diabetes across this large region. A large, and variable, burden of dysglycaemia exists, especially in Middle Eastern and North African countries, associated with high levels of obesity and sedentariness, with a generally lower prevalence in most other parts of Africa. The design and size of studies are highly variable, and more research to quantify the scale of the problem is needed. Local barriers to care relating to issues concerned with gender, consanguinity, lack of understanding of diabetes, lack of understanding of obesity as a health issue, and limited resource at a national level for tracking and intervention for diabetes and other non-communicable diseases. Lifestyle interventions with proven local cost-effectiveness, enhanced access to pharmacologic intervention, and societal interventions to promote better diet and more activity will be an important element in strategies to combat these adverse trends.

OBJECTIVE: Accumulated evidence reported a link between the immune system, microbial infection, and the development of atherosclerosis. Excess intake of high-fat diet (HFD) increases blood lipid levels and induces inflammatory pathways whereas zymosan A (Zym), a microbial component, mediates inflammatory response through the stimulation of specific ligand of toll-like receptors (TLRs) of the immune system. The current research work was aimed to evaluate the mechanism behind atherosclerosis mediated by HFD and Zym in C57BL/6 mice. MATERIALS AND METHODS: The mice were orally fed with HFD for 30 days and Zym (80 mg/kg, single intraperitoneal injection on day 8 th ). On the 31 st day, blood was withdrawn from overnight fasted mice by tail vein puncture and estimated for serum lipids and tumor necrosis factor-alpha (TNF-α). Animals were sacrificed, and cardiac, liver, and aortic tissues were isolated for the estimation of cardiac TLR-2, nuclear factor-kappa B (NF-ƙB); hepatic low-density lipoprotein receptors (LDLR); and base of aorta analyzed for histopathology. RESULTS: It was found that HFD and Zym administration increased arterial inflammation directly through modulation of the TLR-2/NF-ƙB pathway, thereby upregulate serum TNF-α, cardiac TLR-2, and NF-ƙB levels. Further, HFD and Zym treatment significantly increased serum lipid levels and marked decrease in LDLR protein expression in the liver when compared to normal control mice. Histopathological analysis showed the formation of atherosclerotic plaque. CONCLUSION: The study is first, to our current knowledge, to demonstrate the involvement of the TLR-2/NF-ƙB signaling pathway in atherosclerosis induced by HFD and Zym in C57BL/6 mice, resulting in increased degradation of LDLR protein, thereby, increasing the serum lipid levels.
