Cardiopulmonary bypass leads to a preferential loss of activated plateletsA flow cytometric assay of platelet surface antigens

Abstract: Our data suggest that heparinization induces platelet activation. We assume that a loss of larger and more activated platelets from the circulation contributes substantially to the platelet function defect of CPB.

“…On the other hand, avoidance of haemorrhage is important, because haemostasis is vital during and after ECMO [10] and CPB [2,11]. There is evidence that thrombocytopenia due to consumption of activated platelets within pump oxygenator circuits [12,13], and, even more important, platelet dysfunction [2,[13][14][15]] play a central role in the pathogenesis of haemorrhagic complications after ECC.…”

“…It appears that preoperative platelet inhibition reduces the platelet loss associated with CPB because a preferential loss of activated platelets is observed during bypass [21,22]. Muriithii et al [23,24] demonstrated that platelet macroaggregation is virtually abolished during CPB, and suggested that additional platelet suppression is unlikely further to impair hemostasis or increase bleeding during the immediate postoperative period.…”

“…A direct platelet effect of UFH (3–8 U/ml) was suggested by studies that showed the fall in platelet count to begin between heparinization and the first passage of CPB (Mazer et al , 1995; Wahba et al , 1996).…”
