Citrus medica L. commonly known as Otroj, is an important medicinal plant reputed for its nutritious and therapeutic uses. The present work was undertaken to investigate the protective effect of the ethanolic extract of otroj (EEOT) against isoproterenol (ISO)-induced cardiotoxicity in rats. In addition, the antioxidant activity and the phenolic and flavonoidal contents were determined. Rats were administered EETO (250 and 500 mg/kg) or vehicle orally for 15 days along with ISO (85 mg/kg, s.c.) on the 14th and 15th day. ISO induced cardiac dysfunction, increased lipid peroxidation and alteration of myocyte-injury specific marker enzymes. ISO also showed an increase in levels of plasma cholesterol, triglycerides (TG), LDL-C, and VLDL-C. Moreover, the histological investigations showed myocardial necrosis and inflammation. EETO treatment brought the above parameters towards normal level. Moreover, in vitro DPPH radical scavenging and β-carotene-linoleic acid tests of the EEOT exhibited a notable antioxidant activity in both assays used. In addition, histopathological examination reconfirmed the protective effects of EEOT. Thus, the present study reveals that C. medica alleviates myocardial damage in ISO-induced cardiac injury and demonstrates cardioprotective potential which could be attributed to its potent antioxidant and free radical scavenging activity.

Garlic preparations and propranolol (PRO) are agents recognized as cardioprotective and potent antihypertensive agents when they are used individually. However, there is no report available to explain the role of combined therapy during simultaneous hypertension and myocardial damage in rats. We aimed to determine the pharmacokinetic and pharmacodynamic interaction of PRO with garlic homogenate (GH), in rats. The influence of garlic on pharmacokinetics of PRO was determined by HPLC method; while pharmacodynamic interaction was studied in animals with hypertension (10% fructose) and myocardial damage (isoproterenol, 175 mg kg−1, s.c. 2 days). PRO was given orally at 10 mg kg−1 for 1 week, whereas, GH was administered at three different doses of 125, 250 and 500 mg kg−1, p.o. in their respective groups during fourth to sixth week of high fructose (HF) period, once daily. Systolic blood pressure (SBP), heart rate, cholesterol, triglycerides, glucose, creatine phosphokinase-MB, lactate dehydrogenase, superoxide dismutase and catalase were measured and histopathological studies were carried out. The bioavailability and half life of PRO were significantly enhanced by 2- and 3-fold, respectively, in animals pretreated with garlic (250 mg kg−1). Administration of PRO and low to moderate doses of GH (125, 250 mg kg−1), either alone or together showed fall in fluid intake and body weight. The combined therapy of GH 250 mg kg−1 and PRO was found to be most effective in reducing SBP, cholesterol, triglycerides and glucose. These observations suggest that careful addition of garlic in moderate doses in PRO regimen might result in beneficial effect during treatment of hypertensive animals with myocardial damage.

Background: Cardiotoxicity can be developed due to prolonged use of higher doses of some drugs, exposure to some chemicals, toxins or infectious agents and also by some disease conditions. Natural plant food such as peanut (Arachis hypogaea L.) may have free radical scavenging and lipid lowering activity, thereby, can be used for the prevention and management of heart disease. Objective: To observe the protective effect of peanut (Arachis hypogaea L) and its combined action with propranolol on dyslipidemia in Isoproterenol induced cardiotoxic rats. Methods: This experimental study was carried out in the Department of Physiology, Sir Salimullah Medical College (SSMC), Dhaka from January to December, 2012. Twenty Wistar albino rats, age 85-100 days, weighing 120 to 150g (initial body weight) were included in the experimental group (Group B, with peanut). They were further sub-divided into group B 1 (isoproterenol treated group after peanut treatment), and group B 2 (isoproterenol treated group after combined treatment of peanut and propranolol). Age and weight matched 30 Wistar albino rats without any peanut supplementation was taken as control (group A) and divided into three sub-groups, group A 1 (baseline control), group A 2 (isoproterenol treated control) and group A 3 (isoproterenol treated control after propranolol treatment). Each subgroup consisted of 10 rats. After taking final body weight all the rats were sacrificed on 22 nd day. Blood was collected from heart and supernatant serum was preserved in deep freeze until analysis. For assessment of lipid profile status, serum total cholesterol (TC), low density lipoprotein cholesterol (LDL-C) and high density lipoprotein cholesterol (HDL-C) were estimated by standard method. The statistical analysis was done by one way ANOVA and Bonferroni test as applicable. Results: In this study, percent change from initial body weight to final body weight was significantly (p<0.01) lower both in isoproterenol treated group after peanut treatment and in isoproterenol treated group after combined treatment of peanut and propranolol as compared to that of baseline control. Again, this value was almost similar and the difference was not statistically significant between isoproterenol treated group after peanut treatment and isoproterenol treated group after combined treatment of peanut and propranolol. Again, the mean serum TC (p<0.01) and LDL-C (p<0.05) were significantly lower in isoproterenol treated group after peanut treatment and isoproterenol treated group after combined treatment of peanut and propranolol in comparison to those of isoproterenol treated control group. Moreover, the mean serum HDL-C was significantly (p<0.01) higher in isoproterenol treated group after combined treatment of peanut and propranolol, in comparison to that of isoproterenol treated control group.
