Cardiac tamponade: an uncommon presenting feature of systemic lupus erythematosus, a case-based review

Abstract: Although pericarditis is the most prevalent cardiac involvement in systemic lupus erythematosus (SLE), cardiac tamponade is extremely infrequent notably as the first manifestation of the disease. Here we report the case of a 22-year-old woman presenting with cardiac tamponade as the initial presentation of SLE.

“…Pericardial effusion is often small and hemodynamically insignificant in SLE [4]. Despite the prevalence of pericarditis, cardiac tamponade is thought to affect less than 1% of SLE patients, making it a rare and uncommon condition [10,11]. It is a strikingly unusual occurrence as an initial manifestation of SLE.…”

“…The clinical manifestations of tamponade are brought on by a decrease in cardiac output and systemic venous congestion. Cardiac tamponade as the first sign of SLE is very rare and only limited to case reports and series [6,10,13,14]. Typical symptoms and signs include dyspnea, orthopnea, chest pain, pulsus paradoxus, jugular venous distension, and hypotension [3,7].…”

“…When a genetically vulnerable person is exposed to environmental variables, a break in their tolerance results in the activation of their autoimmune system. The immune system is exposed to self-antigens as a result of cell damage brought on by infectious agents and other environmental variables [9]. This activation of T and B cells causes a persistent immunological response that is specifically directed against the damaged cells.…”

“…Heart-related issues will affect around half of SLE patients. The most common cardiac problems in SLE patients are pericarditis, myocarditis, valve issues, thrombosis, and anomalies of the conduction system [10]. Pericarditis, which has been associated with poor mortality, is the most frequent cardiac symptom of SLE.…”
