Cardiac sarcoidosis: Diagnostic, prognostic, and therapeutic considerations

Abstract: Cardiac involvement in patients with sarcoidosis is an important consideration for those who are concerned with this strange disease. Sarcoidosis is not an acute malignant disease but may be noticed at the time of sudden, expected death as fatal myocardial sarcoidosis at autopsy. Even with modern advances in our ability to diagnose heart disease, cardiac sarcoidosis is still often overlooked because of its subclinical disease progression. In view of this, an extensive review of previously published literature … Show more

“…1,4, 20 Corticosteroid therapy has been shown to markedly reduce the size and number of regions of inflammation, 21-23 and myocardial lesions without active inflammation appear to respond poorly to corticosteroids. 24 Thus, it has been proposed that the mechanisms underlying the effects of corticosteroids appear to be related to the attenuation of inflammation associated with granuloma formation in myocardial tissue.…”

“…However, these are often difficult, particularly in cases without any definite extra-cardiac organ involvement. [14][15][16] Actually, it is occasionally experienced that the correct diagnosis of cardiac sarcoidosis is done by the histological examination of myocardial specimens after autopsy or heart surgery. 17 Furthermore, in the course of disease progression, it is sometimes quite difficult to distinguish it from idiopathic dilated cardiomyopathy (DCM).…”

“…Because of the patchy nature of the disease with frequent involvement of the basal intraventricular septum and the left ventricle free wall, false negative results are common [37,38]. Endomyocardial biopsy has a diagnostic yield ranging between 20-30% [39]. According to Diagnostic Standard and Guideline for Sarcoidosis of the Japanese Society of Sarcoidosis and other Granulomatous Disorders, early endomyocardial biopsy is recommended whenever the diagnosis of CS is suspected.…”
