Cardiac resynchronisation therapy in chronic atrial fibrillation: impact on left atrial size and reversal to sinus rhythm

Abstract: Objective: To evaluate the impact of long term cardiac resynchronisation therapy (CRT) on left atrial and left ventricular (LV) reverse remodelling and reversal to sinus rhythm (SR) in patients with heart failure with atrial fibrillation (AF). Patients: 74 consecutive patients (age 68 (8) years; 67 men) with advanced heart failure and AF (20 persistent and 54 permanent) were implanted with a CRT device. Main outcome measures: Patients were evaluated clinically (New York Heart Association (NYHA) class, quality … Show more

“…Third, although 6MWD can be affected by a variety of factors related and unrelated to cardiopulmonary status (27), our analysis demonstrates that the association between ILA and reduced 6MWD is robust to adjustments for many potential confounders. The validity of our findings is additionally supported by our multivariate model, which includes many well-recognized factors associated with reduced 6MWD (e.g., obesity [28], resting heart rate [29], osteoarthritis [30], and atrial fibrillation [31]). Finally, ambulatory oxygen saturation was not measured during the 6MWT in COPDGene.…”

“…Recent studies have suggested that CRT is associated with favorable reverse remodeling effects on the left atrium (LA), in addition to its well-established effects on the left ventricle (LV), thereby possibly reducing the risk for the development of AT and AF in patients with HF (9,10). Despite this, data on the effect of CRT on the resolution of chronic and prevention of new atrial arrhythmias are conflicting (11)(12)(13)(14)(15), possibly because the majority of prior studies had a nonrandomized or retrospective design and comprised relatively small sample sizes.…”

“…Published studies of the effects of CRT in patients suffering from AF are few (15)(16)(17)(18)(19). The MUSTIC (Multisite Stimulation in Cardiomyopathies) AF study, which compared the effects of single-site RV versus biventricular stimulation in patients with advanced CHF, depressed LV function, and a wide RV-paced QRS complex showed a (18) observed that the benefits conferred by CRT in candidates for CRT presenting with AF were limited to patients who had previously undergone ablation of the AV node.…”

“…The MUSTIC (Multisite Stimulation in Cardiomyopathies) AF study, which compared the effects of single-site RV versus biventricular stimulation in patients with advanced CHF, depressed LV function, and a wide RV-paced QRS complex showed a (18) observed that the benefits conferred by CRT in candidates for CRT presenting with AF were limited to patients who had previously undergone ablation of the AV node. In 74 patients with permanent AF and a slow ventricular rate, Kiès et al (19) found that CRT caused a significant decrease in LV end-systolic, LV end-diastolic, and left atrial diameter, and a significant increase in LVEF. The significant improvement in QOL, increase in distance covered during the 6-MHW test and in LVEF, and decrease in LVESV observed after 3 months of standard biventricular stimulation in our study are consistent with the results of these previous studies.…”
