Cardiac involvement in Churg-Strauss syndrome: a follow-up of three cases

Abstract: Three cases of Churg-Strauss syndrome with myocardial involvement are reported. Cardiac disease is known to be the major cause of death in Churg-Strauss syndrome. Aggressive therapy (steroids and cyclophosphamide) may cure the myocardial dysfunction associated with the disease.

“…In some patients with EGPA, mononeuritis multiplex, heart failure, or antineutrophil cytoplasmic antibody (ANCA)-associated systemic vasculitis do not respond to the combination therapy of corticosteroids (CS) and cyclophosphamide (CYC) 3,4 . We reported 5 that intravenous immunoglobulin (IVIG) therapy improved severe mononeuritis multiplex or heart failure in patients with EGPA who did not respond to the combination therapy.…”

“…Corticosteroids and immunosuppressants, especially CYC, have improved considerably most symptoms of vasculitis, particularly those involving the gastrointestinal system, skin, and lungs, as well as constitutional symptoms. However mononeuritis multiplex and heart failure in patients with CSS or ANCA-associated systemic vasculitis do not respond to combination therapy with CS and CYC (Abril et al, 2003;Hattori et al, 1999;Renaldini et al, 1993;Sehgal et al, 1995). IVIG therapy has been used to treat various diseases, including MPA, WG, and ANCA-associated systemic vasculitis (Jayne et al, 1991;Lockwood, 1996;Richter et al, 1995).…”

“…Five of the 13 patients initially reported by Churg & Strauss (1) had heart failure. The myocardial dysfunction may be improved by a therapy combining steroids (73) and cyclophosphamide (74). Heart, or heart and lung transplantation has been proposed (72, 75), but the lack of control of the systemic vasculitis may lead to the recurrence of the disease in the transplanted heart (76).…”
