Abstract1‐Hydroxy‐2.4.5‐trimethylimidazol‐N‐oxid‐(3) III bildet in Chloroform Assoziate, in denen benachbarte Moleküle durch Wasserstoffbrückenbindungen verknüpft sind. Untersuchungen des Teilchengewichts, der Viskosität der Lösungen und der Infrarotspektren haben ergeben, daß die Assoziate bei niedrigen Konzentrationen als zyklische Trimere, bei großen Konzentrationen als kettenförmige Polymere hoher Zähligkeit vorliegen. Monomere Moleküle konnten im entassoziierenden Lösungsmittel Chloroform auch bei großer Verdünnung nicht beobachtet werden. Dadurch wird auf die große Stabilität der Assoziate und der Brückenbindungen hingewiesen.Das NMR‐Signal des Protons der Wasserstoffbrückenbindung beobachtet man bei den hochmolekularen Assoziaten bei extrem niedrigem Feld, 17,3 ppm gegenüber TMS in Richtung kleiner Feldstärke verschoben. Die Wasserstoffbrücke zeigt im IR‐Spektrum mehrere sehr breite Absorptionen. Das Maximum der OH‐Valenzschwingungsbande liegt bei etwa 1400‐1500 cm−1. Es ist gegenüber der monomeren OH‐Bande um etwa 2100 cm−1 nach langen Wellen verschoben. Aus IR‐Daten läßt sich der O‐O‐Abstand der Brückenbindung zu RO‐O ≈︁ 2,4 Å abschätzen. Der ungewöhnlich kurze O‐O‐Abstand macht es wahrscheinlich, daß das Proton in der Wasserstoffbrückenbindung entweder in der Mitte zwischen den beiden Sauerstoffatomen angeordnet ist oder sich in einem Doppelminimumpotential mit sehr niedriger Potentialschwelle bewegt.

Point-of-care ultrasonography (POCUS) refers to the use of portable imaging performed by the provider clinician at the bedside for diagnostic, therapeutic, and procedural purposes. POCUS could be considered an extension of the physical examination but not a substitute for diagnostic imaging. Use of POCUS in emergency situations can be lifesaving in the NICU if performed in a timely fashion for cardiac tamponade, pleural effusions, pneumothorax, etc, with potential for enhancing quality of care and improving outcomes. In the past 2 decades, POCUS has gained significant acceptance in clinical medicine in many parts of the world and in many subspecialties. Formal accredited training and certification programs are available for neonatology trainees as well as for many other subspecialties in Canada, Australia, and New Zealand. Although no formal training program or certification is available to neonatologists in Europe, POCUS is widely available to providers in NICUs. A formal institutional POCUS fellowship is now available in Canada. In the United States, many clinicians have the skills to perform POCUS and have incorporated it in their daily clinical practice. However, appropriate equipment remains limited, and many barriers exist to POCUS program implementation. Recently, the first international evidence-based POCUS guidelines for use in neonatology and pediatric critical care were published. Considering the potential benefits, a recent national survey of neonatologists confirmed that the majority of clinicians were inclined to adopt POCUS in their clinical practice if the barriers could be resolved. This technical report describes many potential POCUS applications in the NICU for diagnostic and procedural purposes.

El embolismo aéreo es una complicación que pocas veces se documenta en las unidades de cuidados intensivos neonatales. Es secundario a la rotura alveolar, colocación de catéteres umbilicales, compresiones torácicas así como a la administración de medicamentos. Se han identificado más casos con la implementación de programas de la utilización de ultrasonido en el punto de atención. Caso clínico: Prematuro extremo de 30 semanas con dos evaluaciones por ultrasonido: el primero en el momento del ingreso porque presentaba hipotensión, encontrándose circulación de transición con adecuada función biventricular y gasto cardiaco bajo. El segundo estudio se realizó posterior a colapso súbito tras la manipulación de catéteres umbilicales; observándose imagen hiperecogénica con sombra acústica en "cola de cometa" en el tracto de salida del ventrículo derecho que generaba obstrucción. Se descartó tamponade, neumotórax y derrame pleural. A pesar del manejo intensivo, el paciente desarrolló acidosis metabólica grave, hiperlactatemia e hipotensión refractaria a aminas, falleciendo posteriormente. Conclusión: El embolismo aéreo es una complicación grave que puede pasar desapercibida y ser potencialmente mortal. El ultrasonido parece ser una buena herramienta para su detección, que ayudará a establecer un manejo oportuno.
