Carcinoma of the gallbladder

Abstract: A retrospective study of 100 proved cases of primary carcinoma of the gallbladder admitted to the University Surgical Unit, University of Hong Kong, over a 20-year period was undertaken. The female to male ratio was 1.4 to 1. The peak incidence was in the seventh decade for the females and the sixth decade for the males. Preoperative diagnosis was made in 10 patients. "Curative" cholecystectomy was performed in 20 patients with a 5-year survival rate of 10 per cent. Palliative procedures were performed in 44 p… Show more

“…One of the major differences between gallbladder and colorectal cancer surgery is that in the latter the tumors are known preoperatively and thereby handled using oncologic principles. This is in contrast to gallbladder cancer, which has been thought to be seldom known preoperatively 28. However, our study on open cholecystectomies showed that the indication for surgery was “suspicion of malignancy” in 40% of the cases and “jaundice” or “acute cholecystitis” in another 44%.…”

“…The causal pathogenesis of the initial cell transformation is still unknown and probably manifold [2, 561. KOO et a1 [56] assume chemical carcinogens to be important etiological factors in gallbladder carcinoma, with gallstones playing a supporting role in the sense of a syncarcinogenesis. Therefore the formation of gallbladder carcinoma is undoubtedly based on the result of an inter-action between a local malignant process and the whole organism [65].…”

“…Metastases from gall bladder carcinoma commonly affect the local areas such as liver, duodenum and the para-aortic lymph nodes [3]. Although the atypical presentations and unusual associations of carcinoma gall bladder are known [4], but initial clinical presentation with distant metastasis is rare [1,5].…”
