Objective: Breast carcinoma en cuirasse is an aggressive manifestation of breast cancer, and is most often seen as a local recurrence. It is the most common primary malignancy to metastasize to the skin and palliative care is often the only option. Methods: Retrospective review of the patient's chart was done. Results: Our patient is a 65 year old female with no significant past medical or surgical history, who presented with a 2 year history of an enlarging left breast mass and left axillary mass, to the point where she had pain with breathing and restriction of chest wall expansion. Work-up of her breast and axillary masses showed carcinoma en cuirasse. Given the advanced stage of her disease, the patient was offered palliative care, which she declined. Conclusions: Breast carcinoma en cuirasse is a rare manifestation of cutaneous metastases from breast cancer. While it is usually seen as a local recurrence, it can also be seen, as in our patient's case, as a natural progression of breast cancer that is difficult to treat. Our patient's case highlights the importance of early detection and treatment of breast cancer, as well as the surveillance for local recurrence.

Cutaneous metastases from solid primary tumours are uncommon. Carcinoma of the breast is the commonest malignancy to metastasize to skin and is usually associated with a local recurrence post therapeutic mastectomy. There are 8 types of cutaneous metastasis related to breast carcinoma which are differentiated on histopathology. Carcinoma en cuirasse, a subtype, clinically presents as a diffuse indurated plaque occasionally accompanied by firm nodules. It characterized by a dense stromal fibrosis and reduced vascularity as well as single filed interstitial tumour infiltrates. Here we report a case of carcinoma en curaisse in a patient with metastatic breast carcinoma.

Carcinoma en cuirasse is a form of cutaneous metastasis of breast carcinoma. Although a rare condition, it is most commonly associated with breast carcinoma with local recurrence after mastectomy and metastasis occurs a few months to years after primary has been diagnosed. Less frequently a metastasis is diagnosed at the same time as primary tumour is diagnosed. We report a case of carcinoma en cuirasse in a 60 year old female who presented with elevated nodular indurated skin lesions on anterior and posterior left chest wall and axillary region. FNAC of lesions was performed. It was diagnosed as metastatic deposits of Duct Cell Carcinoma. It was followed by histopathology and the diagnosis was confirmed. Hence it is very important to investigate thoroughly keloid like or indurated patches on skin of long duration.
