Neuroendocrine cell proliferations in lungs explanted for fibrotic interstitial lung disease and emphysema

Using recently proposed pathological criteria, we determined the incidence of neuroendocrine cell proliferation in a series of explants with lung disease. Cases were defined as NECH (3 bronchioles with 5 endocrine cells), borderline diffuse neuroendocrine cell hyperplasia (DPNECH) (1-3 tumourlets with or without NECH), and DPNECH (3 tumourlets with NECH). Endocrine cells were identified by immunohistochemical staining for synaptophysin. There were 65 explants with interstitial lung disease (57 with non-sarcoid fibrotic lung disease, 8 with sarcoidosis), and 21 with centrilobular emphysema. Over one-third of all explant cases demonstrated histological criteria for NECH. There were three cases of DPNECH in the non-sarcoid fibrotic lung disease group (5%) and 20 cases of NECH (35%). The emphysema group had one case of DPNECH (5%), two cases of borderline DPNECH (10%), and seven cases with NECH (33%). The sarcoidosis group had two cases of DPNECH (25%) and three cases of NECH (38%). NECH is common in interstitial lung disease and emphysema. These results suggest that fibrotic lung disease is a predisposing factor for neuroendocrine cell proliferation, in addition to the known risk of epithelial neoplasms.

Among these, 5 patients with an association between carcinoid tumors and sarcoidosis have been documented in the literature [9]. Two of them had a concurrent diagnosis of both diseases, 1 patient had an intestinal carcinoid tumor 3 years before the diagnosis of sarcoidosis was made, 1 patient 40 years before the diagnosis, and 1 patient was operated on for a carcinoid tumor of the appendix 1 year after the manifestation of sarcoidosis.…”

Background: The association of malignant tumors and sarcoidosis is uncommon in clinical practice. It has been supposed, but not clearly demonstrated, that the two entities may be etiologically related. Case Report: A patient with a rectal carcinoid tumor and a bilateral hilar enlargement due to sarcoidosis is described. The patient had no specific symptoms from the endocrine tumor, and sarcoidosis was a coincidental finding. He underwent a low anterior rectal resection. Histological examination of the specimen revealed a T3–4 N0 carcinoid tumor, and sarcoidosis was proven by mediastinal biopsy. The patient received no further therapy for sarcoidosis. One year postoperatively the patient has no evidence for recurrent disease or metastasis, but bilateral hilar enlargement due to sarcoidosis is present even now. Conclusion: Only 4 patients with sarcoidosis and intestinal carcinoid tumor are reported in the literature. This is the first report of a patient with rectal carcinoid tumor associated with sarcoidosis. We emphasize the importance of mediastinal biopsy in patients with cancer and associated hilar lymphadenopathy in order to avoid the diagnosis of disseminated malignant disease.

Sarcoidosis is an inflammatory disease of unknown cause which is characterized by the presence of noncaseating granulomas. The association of sarcoidosis with malignancy has been an issue and remains controversial. We herein report endobronchial carcinoid tumor in a patient with sarcoidosis diagnosed by nasal mucosal biopsy, excisional biopsy of cervical lymph nodes and mediastinal lymph node dissection.
