Carcinoid heart disease: diagnosis by two-dimensional echocardiography.

Abstract: We prospectively studied 14 patients with the carcinoid syndrome to determine if two-dimensional echocardiography could detect the nature and extent of valvular abnormalities. Eight of the 14 patients had definite abnormalities of the right-sided cardiac valves. The tricuspid valve had a characteristic appearance, similar to previously described pathologic findings. The leaflets appeared diffusely thickened, shortened and stiff without evidence of commissural fusion. Saline contrast studies demonstrated tricus… Show more

“…The development of more sensitive investigational methods has led to the realization that cardiac lesions are probably TABLE 3 present more often in patients with malignant carcinoid tumors than previously believed. [1][2][3][4][5][6] Malignant midgut (jejunum, ileum and proximal colon) carcinoid tumors release vasoactive substances such as serotonin, histamine, bradykinin, kallikrein, prostaglandins, and tachykinins.30-34 Some of these have been suggested as possible etiologic agents for the carcinoid heart disease. This study of 68 well-characterized patients with malignant midgut carcinoid tumors reveals a significantly higher urinary 5-HIAA excretion in patients with severe carcinoid heart disease than in those without or with a lesser degree of right heart abnormalities.…”

“…The excess of hormones such as 5-HT, substance P, and bradykinins released into the circulation (Mirtensson rt ul., 1985;Nobin et al, 1982Nobin et al, , 1983 cause premature death from cardiac and nutritional failure. In fact, cardiac complications have been claimed to be one of the most important lethal factors in carcinoid syndrome (Costello, 1975;Robertson and Sjoerdsma, 1964 (Howard et al, 1982). They investigated and followed 14 patients with carcinoid syn- drome and, although 8 patients had definite abnormalities of the right-sided cardiac valves, no patient died as a direct result of carcinoid heart disease.…”
