O ferro é um elemento essencial na maioria dos processos fisiológicos do organismo humano, desempenhando função central no metabolismo energético celular. 1,2 Deficiência de ferro é a alteração hematológica mais comum e acomete 20% a 30 % da população mundial, sobretudo crianças menores de 3 anos de idade e mulheres em idade fértil, constituindo-se, portanto, num grave problema de saúde pública, particularmente nos países em desenvolvimento. [3][4][5] A deficiência de ferro resulta do desequilíbrio entre quantidade absorvida e consumo e/ou perda e deve-se a diversos fatores, tais como: Fisiológicos (necessidade aumentada -crescimento, gestação, parto e puerpério); Nutricionais (dieta não balanceada, ou seja, baixa disponibilidade de ferro heme; uso de antiácidos, ingesta de fitatos, fosfatos, oxalatos e tanino, que diminuem a absorção de ferro); Patológicos (perda de sangue ou diminuição de absorção intestinal -menorragia, sangramento gastrointestinal, parasitose, intolerância à lactose, medicamento [aspirina, anti-inflamatório não hormonal, anticoagulante], doação de sangue, gastrectomia, gastroplastia redutora. [6][7][8][9] A deficiência de ferro desenvolve-se de maneira lenta e progressiva e, didaticamente, pode ser dividida em três estádios: depleção dos estoques de ferro, eritropoese deficiente em ferro e anemia ferropênica, que corresponde ao terceiro e último estádio da deficiência de ferro e caracteriza-se por diminuição da oferta de ferro à medula óssea com redução da síntese e do conteúdo de Hb nos precursores eritrocitários. 2 Neste estádio, observa-se, além da redução da concentração da hemoglobina, hipocromia, microcitose, anisocitose e poiquilocitose; ferro sérico < 50 µg/dl; saturação da transferrina < 20% e ferritina < 30 ng/ml. 2,5,6,7 O tratamento da anemia ferropênica compreende orientação nutricional e reposição na dose correta e por tempo adequado de ferro, além da identificação e correção, quando possível, da causa que levou à anemia.A via preferencial de reposição de ferro é a oral, e a dose terapêutica recomendada é de 2 a 5 mg/kg/dia por período suficiente para normalizar os valores da Hb (quatro a oito semanas) e restaurar os estoques normais de ferro do organismo (dois a seis meses ou até obtenção de ferritina sérica maior que 50 ng/ml). 2,5 Na prática, a dose máxima de ferro preconizada é de 150 a 200 mg de ferro elementar por dia, dividida em duas ou três tomadas. Acima de 200 mg, a mucosa intestinal atua como barreira, impedindo a absorção deste metal, e a proporção de ferro absorvido diminui significativamente. 2,5,8 As causas mais frequentes de falha no tratamento com ferro oral são: continuidade da perda de sangue (por falha na identificação de sangramento e/ou de distúrbio de absorção de ferro [doença celíaca, gastrite atrófica autoimune e infecção pelo helicobacter pylori]); medicação usada inadequadamente (baixa adesão ao tratamento devido aos efeitos adversos (EA) gastrointestinais e/ou dose inadequada e/ou duração insuficiente); doença coexistente interferindo na resposta ao ...
