Características clínicas y factores pronósticos de la enfermedad meningocóccica: un estudio de serie de casos en Chile durante el brote 2012-2013

Abstract: The case fatality rate exceeded normal levels and was higher in serogroup W. Increasing in this serogroup, associated to the increased presence of nonspecific symptoms or rapid progression to septicemia, hit a health system accustomed to more classic meningococcal disease presentation, which could partly explain the observed increased fatality rate.

“… 32 , 33 , 34 , 35 , 36 Previously reported case-fatality rates of invasive MenW disease varied between 0 and 30%. 33 , 35 , 36 , 37 In the time-frame of our dataset, a capsular switch of cc11 occurred from MenC to MenW, a well-known phenomenon. 38 , 39 In our previous study, clonal complex 11 was only associated with serogroup C and also associated with unfavourable outcome, which might indicate that clonal complex 11 has more impact on outcome than the serogroup.…”

“…31 In the last decades, publications on molecular information in Latin America have increased, evidencing that in Colombia, cc ST-9493 was observed in MenB between 2013 and 2016 32 and in Chile, since 2011, there was an increase in cases of IMD due to cc ST11, with predominance over the other cc during 2012 and 2013. 17,33,34 Most MenW cc ST-11 strains in this study had an antigen profile: W:P1.5,2:ST-11 (cc11) and fHbp allele 22, which differs from MenW strains identified in Brazilian in recent years (W:P1.5,2:ST-11) with a novel fHbp peptide 1241, 35 or fHbp peptide 24 in MenB. 36 Our study shows a correlation between the clinical manifestations, outcome, and genotypic characterization of N. meningitidis, accounting for the 3 main circulating cc in our country during the 2010-2018 period, with a predominance of the cc ST11.…”

“…Asimismo, la vigilancia puede gatillar medidas de salud pública ante eventuales brotes, tales como la introducción en 2014 de la inmunización contra N. meningitidis serogrupo W tras la precoz identificación de un brote en 2012 13,29 . En nuestra serie no hubo un cambio sustancial en el número de cultivos de LCR ni episodios de meningitis por N. meningitidis serogrupo W en el año 2012 probablemente por la exclusión de la población pediátrica, pero sí se identificó un caso en el año 2014 30 .…”
