Capsular tension ring implantation after capsulorhexis in phacoemulsification of cataracts associated with pseudoexfoliation syndrome

Abstract: In cases of cataract associated with pseudoexfoliation syndrome, implanting a CTR before phacoemulsification of the nucleus reduced intraoperative zonular separation, increased the rate of capsular IOL fixation, and improved UCVA.

“…The Morcher CTR is indicated for zonular dehiscence less than 4 clock hours (traumatic or iatrogenic), generalized zonular weakness (idiopathic, pseudoexfoliation syndrome), or lens subluxation (idiopathic, trauma, and Marfan's and Weill-Marchesani syndromes). [1][2][3][4][5][6][7] The intraocular lens (IOL) position has also been shown to stabilize. 8 To our knowledge, no study has investigated the effect of a CTR on refractive outcome.…”

“…[3][4][5][6][7][8][9] It has also been shown in most accounts to limit capsular bag phimosis, thus preventing intraocular lens (IOL) decentration, tilting, and deformation. 10 The goal of this study was to gain a better understanding of the anatomic relationship of the Morcher CTR to posterior segment structures (ie, posterior iris, ciliary body) and the posterior chamber IOL using ultrasound biomicroscopy.…”

“…5 After surgery, a CTR helps maintain the equatorial area of the lens capsule in a fully distended circle of 360 degrees, which minimizes the risk for decentration of the intraocular lens (IOL) and may decrease the risk for dislocation of the capsule-IOL complex. 5,6 One challenge in using a CTR is the difficulty of removing nuclear or cortical material once the ring has been implanted. The circular ring compresses residual lenticular/cortical material against the capsular bag and impedes its removal.…”
