Capsular retraction in hepatic giant hemangioma: CT and MR features

Abstract: We report two cases of hepatic giant hemangiomas with capsular retraction of the liver adjacent to the tumor on computed tomography and magnetic resonance images. Our cases show that the retraction of the liver capsule adjacent to the tumor is not a finding specific to malignant hepatic tumors but can also be observed in benign hepatic tumors.

“…Capsular retraction -This alteration has been described as a sign of malignancy in focal hepatic lesions (28)(29)(30) . However, in at least one case of our study, it was possible to identify a peripheral hemangioma associated with capsular retraction, as already described in the literature (3,31,32) . In these cases, the fibrosis associated to the hemangioma peripheral localization may be responsible for the capsular retraction (31) .…”

“…However, in recent reports, this finding has also been found in hepatic haemangioma, which is the most common benign tumour of the liver [5,6]. Capsular retraction associated with hepatic haemangioma is due to central thrombosis and fibrosis of large vascular channels of this tumour [6], which is seen more often in larger haemangioma ( Figure 8).…”

All reported cases of haemangioma with capsular retraction were larger than 4 cm and were classified as giant haemangioma [5,6].…”

“…Soyer et al (3) reported that this finding was never associated with a benign tumor, supporting the hypothesis that a retraction of the liver capsule adjacent to a hepatic tumor is a finding specific to malignant tumors. Recently, Yang et al (9) reported a capsular retraction in hepatic giant hemangioma. A capsular retraction with hemangiomas in cirrhotic livers has also been described (10).…”
