Cannula Site Insertion Technique Prevents Incisional Hernia in Laparoscopic Fundoplication

Abstract: Trocar-site hernia is a rare complication of laparoscopy. It occurs at the trocar insertion site with a diameter of 10 mm or more in adult patients. Trocar insertion away from the midline can decline the incidence.

“… 10 , 11 Para-median incisions have also been shown in Level 2 general surgery studies not to require fascial closure when blunt trocars are used ( Table 4 ) . 12 17 The arcuate line may also demarcate an inherent weakness of the abdominal fascia. It is the place where the fascia of the internal oblique and the transversus abdominus of the posterior rectus sheath migrate to join the anterior sheath, leaving only the transversalis fascia and peritoneum for closure support deep to the rectus muscle ( Figures 2 and 3 ) .…”

“…Some studies suggest there may be a greater incidence of herniation in the midline ports rather than off-midline ports 5,8,39 ; however, others suggest this is not the case but rather larger diameter ports and extraction of the gallbladder are more likely to occur at the midline ports. 2,12 …”

“…5 The incidence is estimated to be between 0.65% and 2.8%. 2,[6][7][8][9][10][11] It is pertinent to note that the true incidence may be much higher because patients who are asymptomatic may not seek medical attention unless an extended follow-up protocol has been established. In addition, subfascial hernias could be missed during the clinical examination because follow-up does not include routine imaging.…”

“…In addition, subfascial hernias could be missed during the clinical examination because follow-up does not include routine imaging. 5,6 Although PSH appears uncommon, it could precipitate the downstream complication of hernia incarceration or strangulation requiring emergent surgical intervention. 2 Furthermore, 20% of repaired PSHs tend to recur, imposing significant financial burden on patients.…”
