Candidiasis

Abstract: Candida spp. are the most common fungal pathogens isolated in immunocompromised hosts, particularly cancer patients. Numerous clinical manifestations of candidiasis have been recognized, including localized infection such as oropharyngeal candidiasis or focal hepatic candidiasis, and disseminated infection resulting from hematogenous spread, with or without documented fungemia. Granulocytopenic patients are particularly at risk. Candida albicans is isolated in approximately 40% of cases of fungemia, other Cand… Show more

“…3 Prolonged use of indwelling catheters promotes fungal colonization while providing a vascular portal of entry. 1 Although neutropenia is the most potent predisposing factor in systemic fungal infection, 4 it is not a common complication in patients after OLT. The reticuloendothelial system of the liver is an integral part of the normal host immune defense, and because this physiological sieve does not adequately function early posttransplantation, these patients are at higher risk for bacterial and fungal sepsis than other solid-organ transplant recipients.…”

“…Candida albicans is one of the most frequently encountered etiologic agents that cause opportunistic infections such as so-called candidiasis, a disease whose systemic manifestation could prove fatal and whose incidence is increasing as a result of an expanding immunocompromised population (5,37,39).…”

“…Amphotericin B showed fungicidal activity against C. albicans growing both extracellularly and intracellularly, whereas fluconazole inhibited growth only of extracellular C. albicans. A slight antagonistic effect between fluconazole and aiflphotericin B was found with respect to intracellular as well as extracellular C. albicans.Deep-seated candidal infections are an important cause of morbidity and mortality in immunocompromised patients (15,21,22). The current drug of choice for most systemic mycoses is still amphotericin B (AmB), but its use is restricted by a variety of toxic side effects (6,13,15,21,22).…”
