Candida glabrata Endophthalmitis Treated Successfully with Caspofungin

Abstract: A 39-year-old man with Candida glabrata endophthalmitis was successfully treated with a 28-day course of intravenous caspofungin. Presence of underlying renal insufficiency and infection with a drug-resistant strain precluded use of amphotericin B or fluconazole. Intravitreal administration of antifungals and vitrectomy were not required. The role of caspofungin in Candida endophthalmitis is discussed.

“…As the first of the echinocandins to be approved for the treatment of IC, caspofungin has been used extensively for the treatment of all forms of serious candidal infections over the past 4 years (87,114,134,171,261). Thus far, clinical experience with caspofungin and IC has been good (41,87,114,163,171).…”

“…The potent, broad-spectrum fungicidal activity of caspofungin against Candida spp. has led to extensive use of this agent for the treatment of all forms of serious candidal infections over the past 4 years (9,12,17,25). Thus far, clinical experience with caspofungin and invasive candidiasis has been good (9,14,17).…”

“…The case of a 39-year-old patient with endophthalmitis resulting from complicated abdominal surgery has been published, successfully treated with intravenous caspofungin. 5 A case of endophthalmitis subsequent to urologic surgery has also been published, successfully treated with amphotericin B and flucytosine lipid complex. 7 Treatment regimens for endophthalmitis with fungal etiology include various combinations of systemic or intravitreal antifungals plus vitrectomy.…”

“…3 Although endogenous fungal endophthalmitis has been reported, bilateral ocular involvement is a less common condition. 4 Even less is endogenous endophthalmitis from Candida glabrata [5][6][7] (C. glabrata). There are no papers reporting bilateral endogenous endophthalmitis from C. glabrata.…”
