Background and AimGastric intestinal metaplasia (GIM) is a high‐risk factor for the development of gastric cancer. Narrow‐band imaging (NBI) enables endoscopic grading of GIM (EGGIM). In the era of climate change, gastrointestinal endoscopists are expected to reduce greenhouse gas emissions and medical waste. Based on the diagnostic performance of NBI endoscopy, this study measured the environmental impact and reduced cost of implementing EGGIM during gastroscopy.MethodsUsing NBI endoscopy in 242 patients, EGGIM classification and operative link on GIM (OLGIM) staging were prospectively performed in five different areas (lesser and greater curvatures of the corpus and antrum, and the incisura angularis). We estimated the environmental impact and cost reduction of the biopsy procedures and pathological processing if EGGIM were used instead of OLGIM.ResultsThe diagnostic accuracy of NBI endoscopy for GIM was 93.0–97.1% depending on the gastric area. When a high EGGIM score ≥ 5 was the cut‐off value for predicting OLGIM stages III–IV, the area under the curve was 0.862, sensitivity was 81.9%, and specificity was 90.4%. The reduction in the carbon footprint by EGGIM was −0.4059 kg carbon dioxide equivalents per patient, equivalent to 1 mile driven by a gasoline‐powered car. The cost savings were calculated to be $47.36 per patient.ConclusionsEGGIM is a reliable method for identifying high‐risk gastric cancer patients, thereby reducing the carbon footprint and medical costs in endoscopy practice.

OBJECTIVETo study the association of NDMA-contaminated generic valsartan with cancer incidence.DESIGNNationwide longitudinal observational cohort study.SETTINGClaims data from the National Health Insurance Service of South Korea was analyzed using 1:1:1 pairwise propensity score matching (PSM) of NDMA-uncontaminated original, NDMA-contaminated generic, and initially-suspended-but-finally confirmed as NDMA-uncontaminated generic valsartan user-groups. Time-dependent Cox models and dose- response evaluation were used to evaluate carcinogenicity.PARTICIPANTSA total of 3,231,212 participants with satisfactory minimal adherence, followed up from January 1, 2013 until December 31, 2020.INTERVENTIONSAt least one tablet of valsartan.MAIN OUTCOME MEASURESThe primary outcome was primary cancer incidence and secondary outcome was 12 prevalent organ-specific cancer incidences. All-cause and cardiovascular mortality risks were estimated before and after valsartan withdrawal.RESULTSAmong participants (mean [standard deviation] age 59.5 [13.1] years; male, 53.5%), new users had adjusted hazard ratios and 95% confidence intervals (CI) of 1.069 (1.054 to 1.085) and 1.142 (1.100 to 1.186) for any cancer in the NDMA-exposed period (versus NDMA-unexposed) before and after 1:1:1 PSM, respectively. Regardless of PSM, lung and prostate cancer risks increased significantly during the NDMA-exposed period whereas, post-PSM, cancer risk did not increase in the eventually-NDMA-uncontaminated group, even in new users. All-cause and cardiovascular mortality did not differ significantly with NDMA exposure before and after emergent banning.CONCLUSIONSNDMA-contaminated valsartan increased cancer risk, especially of lung and prostate cancers. All-cause and cardiovascular mortality did not evidently increase following banning. This supports emergent health-policy action against potentially carcinogenic drugs.

Background: Gastric cancer (GC) constitute a significant cause of cancer-related mortality worldwide, with metastatic patterns including hematogenous, peritoneal, and ovarian routes. Although GC gene expression patterns have been extensively researched, the metastasis-specific gene expression landscape remains largely unexplored. Methods: We undertook a whole transcriptome sequencing analysis of 66 paired primary and metastatic (hematogenous, peritoneal, or ovarian) GC tumors from 14 patients. Public databases including The Cancer Genome Atlas (TCGA) and Gene Expression Omnibus (GEO) was used for validation. Single-cell RNA sequencing (scRNA-seq) of four ascites from serosa positive GC patients and five primary tumors by layer (superficial and deep) were analyzed. Results: Through differential expression analysis between paired primary and metastatic tumors by routes identified 122 unique metastasis-specific epithelial-mesenchymal transition (msEMT) genes. These genes demonstrated varying expression patterns depending on the metastatic route, suggesting route-specific molecular mechanisms in GC metastasis. High expression of msEMT genes in primary tumors was associated with more frequent CDH1 mutations, the genomically stable subtype, and poor prognosis in TCGA GC cohort. This association was further corroborated by poor prognosis and high predictive performance for peritoneal/ovarian recurrence in two independent cohorts (GSE66229; n=300, GSE84437; n=433). scRNA-seq analysis of five primary tumors (GSE167297) and four independent ascites samples from GC patients revealed that msEMT genes were predominantly expressed in diverse fibroblast sub-populations, rather than cancer cells. Conclusions: This study illuminates the route-specific mechanisms and underlines the significance of msEMT genes and cancer-associated fibroblasts in peritoneal metastasis of GC.
