Cancer of the gastric stump after partial gastrectomy (Billroth II principle) for ulcer

Abstract: Eighteen cancers of the gastric remnant after resection for peptic ulcer are reviewed. Current views on the etiology and pathogenesis of this malignancy are described, as a prolonged exposure of the gastric mucosa to alkaline juices seems to create a precancerous state. Diagnosis of the condition through symptoms, x‐ray examination, cytology, and endoscopic methods is discussed. The curative procedure is recommended, and the poor value of palliative surgery is emphasized. Seven survivors (three long‐term survi… Show more

“…Carcinoma of the gastric stump as a late complication of resection for benign disease is a well-recognised clinical entity (Saegesser and James, 1972;Morgenstern et al, 1973). There have been a number of reports of an increased incidence of stump carcinoma compared with carcinoma in the normal population (Krause, 1958;Schrumpf et al, 1977), although some authors have drawn the opposite conclusion (Liavaag, 1962).…”

“…An increased risk of gastric cancer in stomach remnants long after partial gastrectomy and possibility of effects of reflux of bile and/or pancreatic juice have been reported 45 . Promoting effects of bile on experimental stomach tumorigenesis in the rat have also been suggested [46][47][48] and it has been shown that the incidence of gastric cancers induced by carcinogens is increased after partial gastrectomy, with tumor development related to the promotional effect of bile reflux 49 .…”
