The management of breast cancer is evolving rapidly. Because of the improvement in treatment of localized breast cancer, the number of patients with metastases has fallen and the features of relapsing disease have changed. Different subtypes of breast cancer have been identified and associated with specific molecular mechanisms and potential therapeutic targets. In order to optimize use of new treatments, it will be necessary to identify and explore the mechanisms of sensitivity and resistance as the disease evolves. In this area, we face some of the many challenges relating to better management of patients with metastatic breast cancer in the next years.Résumé : La prise en charge des cancers du sein est en pleine évolution. Suite aux progrès dans la prise en charge des cancers du sein localisés, le nombre de patientes métastatiques tend à diminuer, et les caractéristiques des maladies en rechute se trouvent modifiées. Différents soustypes de cancers du sein ont pu être identifiés et associés à des mécanismes moléculaires spécifi-ques et des cibles thérapeutiques potentielles. Les nouvelles théra-pies en cours de développement nécessitent d'identifier les méca-nismes de sensibilité/résistance qui, sans doute, devront être éva-lués lors de l'évolution de la maladie pour une meilleure adéquation des traitements. Ce ne sont là que quelques-uns des nombreux challenges pour une meilleure prise en charge des patientes atteintes de cancer du sein métastatique dans les années à venir.
