Survival in rectal cancer has been related mainly to clinical and pathological staging. Recurrence is by far the most challenging issue when surgical treatment of rectal cancer is concerned. This study aims to establish a recurrence pattern for rectal adenocarcinoma submitted to surgical treatment between March 2003 and July 2016. After exclusion criteria were applied, one hundred twenty two patients were analyzed. Global recurrence was found in 22% of them, while 13.1% have had local recurrence. Disease-free survival was 23.9 months, in average, and medium follow-up was 34.13 months, varying from 6 to 115 months. Recurrence, in literature, is usually between 3 and 35% in 5 years, and shows a 5-years survival rate of only 5%. Around 50% of cases, recurrence is local, confined to pelvis. This data followed literature in most aspects evaluated, although finding a high rate of local recurrence remains a challenge in the seek for better surgical outcomes.

Objetivo Descrever os casos de câncer colorretal quanto aos aspectos sociodemográficos e clínicos no estado do Maranhão, Brasil. Métodos Trata-se de um estudo transversal, retrospectivo com abordagem quantitativa. Os dados foram exportados do sistema de informação de registro hospitalar de câncer, com recorte temporal de 2013 a 2017, sendo analisados por estatística descritiva. Resultados Foram analisados 421 casos de câncer colorretal, houve a predominância do sexo feminino com (55,3 %), idade estava entre 60 a 70 anos (29,5 %), a raça/cor das pacientes era parda (67,7 %), o estado civil era casado (46,8 %), escolaridade ensino fundamental incompleto (42,3 %) e ocupação agricultoras (22,1 %). As topografias tumorais mais ocorrentes foram do colón (36,1 %), tipo histológico foi adenocarcinoma (71,3 %), os tumores foram classificados em estádio IV (27,4%) e sem registro de metástase (74,3 %), não apresentavam histórico de alcoolismo e tabagismo, 28 % e 29,2 %, respectivamente. Foram tratados com cirurgia e quimioterapia (19 %), apresentaram remissão da doença/cura (26,8 %) dos casos, não tinham histórico familiar de câncer (23,8 %), associaram múltiplos exames para o diagnóstico (79,1 %) e o tempo entre diagnóstico e o tratamento foi acima de 60 dias (37,5 %). Conclusão O câncer colorretal é um problema de saúde pública devido ao desenvolvimento silencioso da doença, os fatores de risco da população e desafio de mudança no estilo de vida na população. Dessa forma, é necessário implementar estratégias de rastreamento, prevenção e diagnóstico precoce com finalidade de reduzir os impactos da morbimortalidade da neoplasia na população de idosos.

Objective : to evaluate the clinical characteristics of patients with colorectal cancer under the age of 50 treated at a public hospital in Brasilia over 5 years. Methods: we conducted a longitudinal, retrospective study, with 184 patients undergoing surgical procedures at the Asa Norte Regional Hospital (HRAN), including those who underwent only biopsy, between January 2013 and January 2018. We divided the patients into two groups: under the age of 50 (n=39) and age equal to or greater than 50 years (n=145). We compared the groups as to age, sex, symptoms, time between symptom onset and diagnosis, family and personal history, tumor location, histopathological characteristics, applied surgical management, staging and mortality. Results: the group of patients under the age of 50 had more individuals with stage III and IV (p=0.041), more frequent poorly differentiated tumors (10.25% versus 3.52%; p=0.153), and higher incidences of compromised surgical margins (p=0.368), angiolymphatic (p=0.07) and perineural (p=0.007) invasion, which denotes more advanced disease in this group of patients. Conclusions: the study showed the low effectiveness of population screening methods for colorectal cancer currently used in this population, given the high incidence of the disease and late diagnosis in both groups.
