Introdução: O câncer anal é um tumor raro cuja incidência é influenciada pelo comportamento sexual e por condições de vulnerabilidades biológicas e sociais. Uma provável explicação para o aumento da incidência nas últimas décadas em países em desenvolvimento é a baixa abrangência ou inexistência dos programas de rastreamento das lesões precursoras. Objetivos: Abordar os métodos combinados que compõem o tripé básico no Sistema Único de Saúde (SUS) para avaliação do canal anal e diagnóstico do câncer e/ou suas alterações precursoras causadas pelo HPV, orientando a coleta e o processamento técnico das amostras nos serviços públicos de saúde. Métodos: Trata-se de uma revisão seletiva da literatura de naturezas qualitativa e exploratória, utilizando publicações disponíveis na interface pública do Portal de Periódicos Capes. Resultados: Não há programas para rastreio das lesões precursoras do câncer anal, mas a aplicação dos recursos para diagnóstico das alterações iniciais baseia-se no sucesso obtido com o rastreamento das lesões precursoras do câncer de colo uterino, visto que as patologias compartilham semelhanças cito-histológicas, etiológicas, fatores de riscos, técnicas para diagnóstico e profissionais de saúde. Neste artigo revisam-se os métodos diagnósticos disponíveis e as possibilidades de tratamentos das lesões precursoras anais a fim de prevenir a evolução para o câncer. Com esse material, os profissionais de saúde podem se beneficiar com as orientações e, então, exercer relevante atuação na prevenção da neoplasia. Conclusão: É possível incorporar no SUS uma política de diagnóstico precoce para qualificar e expandir as iniciativas de promoção e atenção às pessoas.
