Canaloplasty in Open-angle Glaucoma: Mid-term Results From a Multicenter Study

Abstract: Canaloplasty is a quite difficult surgical technique; however, mid-term results are promising. Complications can sometimes occur, but are seldom serious. The main advantage of this promising bleb-less procedure is that physiological humor aqueous outflow is restored.

“…Additional studies published comparing these two procedures do not include racial demographics. [ 4 5 7 ] Furthermore, it has been proposed that non-Caucasions are at higher risk for bleb failure following trabeculectomy. [ 11 ] Therefore, this study contributes to the current literature 2-year data comparing vision, IOP lowering medication, and mean IOP reduction between these procedures among a multiethnic group, providing broader applications among different patient populations.…”

“…[ 6 ] Additional studies published comparing these two procedures also do not include racial demographics. [ 4 5 7 ] We present 2-year surgical outcomes of canaloplasty and trabeculectomy with mitomycin C (MMC) performed in a multiethnic group with moderate to advanced primary open-angle glaucoma (POAG).…”

“…Some authors have suggested that a filtering bleb is also formed and that subconjunctival drainage occurs in 5-12% of patients following canaloplasty. 4,5,[7][8][9][10][11][12][13] Such blebs develop inadvertently either through seepage of aqueous through the gutters of the scleral flap or incomplete water-tight closure. However, to date, the resulting anatomy reported does not resemble typical filtering blebs.…”

“…7 The success rate of canal cannulation ranges from 74% to 89.9% as reported in a number of different studies. 4,5,8,55 Difficulties associated with intubation may occur in the event of unforeseen anomalies being present within Schlemm's canal in addition to prior laser procedures on the filtration angle (argon laser trabeculoplasty ALT) that have caused destruction of the trabecular meshwork structure and excessive scarring. 56 Patients with angle anomalies, such as angle recession, anterior synechiae, glaucoma congenitum, should also be excluded.…”
