Can Video Endoscopic Inguinal Lymphadenectomy Achieve a Lower Morbidity Than Open Lymph Node Dissection in Penile Cancer Patients?

Abstract: This preliminary series suggests that VEIL can reduce morbidity, including hospitalization times, compared with standard open surgery. Oncologic results are premature but seem similar to the results from the conventional open operation. VEIL is a promising minimally invasive approach for radical inguinal dissection in penile cancer patients with nonpalpable or low-volume palpable inguinal disease.

“…Endoscopic inguinal lymph nodes resection surgery is adopted in the male carcinoma of penis and obtained a better effect (Tobias-Machado et al, 2008;Schwentner et al, 2013;Zhou et al, 2013). Place the laparoscope by piercing through skin below the navel and push it to the inguinal region after it was separating in the fat layer (Tobias-Machado et al, 2008).…”

Or puncture into the subcutaneous fat along the middle of thigh upward, create small lacuna below the subcutaneous fat of inguinal region using the laparoscope and inject CO 2 to form air space and then cut the adipose and lymphoid tissue like traditional surgery.…”

“…16 quienes reportaron una baja tasa de complicaciones. Otros autores posteriormente han reportado igualmente menor morbilidad de la linfadenectomía inguinal videoendoscópica frente al abordaje abierto [17][18][19] . En nuestro caso, la vena safena fue preservada sin presentar ninguna complicación postoperatoria ni a los 3 meses de seguimiento.…”

“…Nodal counts were similar, with 20% of complications on the endoscopic side, compared with 70% with open surgery, p 0.011. Skin-related complications were reduced from 50% to 5% with the endoscopic approach 5% [8]. Sotelo et al [9] reported the outcomes after 14 inguinal endoscopic lymphadenectomies in eight patients with clinical Stage T2 squamous cell carcinoma of the penis, with a median operative time of 91 min and an average node yield of nine.…”

“…In summary, the evidence from small series [6][7][8][9][10], comprising a total of 41 endoscopic groin dissections, [11] suggests that the morbidity of an endoscopic dissection is lower than open contemporary series [3]. A similar number of nodes can be retrieved [7], and more importantly, when an independent surgeon 'audited' the dissection, the field was adequately dissected in 94.7% [18].…”
