Background: Immobilization of the cervical spine is indicated in all patients with the potential risk of any cervical spine injury. Airway management in these patients is challenging and direct laryngoscopy is the standard of care. Videolaryngoscopes like the ET-View were introduced into clinical practice to provide better airway visualization and ease intubation. Although the ET-View is essentially a conventional endotracheal tube, it is equipped with a miniature camera in its tip. The ET-View has not been investigated in patients with immobilized cervical spine so far. The aim was to evaluate the performance of the VivaSight SL compared with Macintosh when performed in patients with immobilized cervical spine by inexperienced physicians. Methods: This was a prospective, randomized, cross-over manikin trial. Fifty novice physicians were randomly assigned to intubate a manikin in three airway scenarios including a normal airway and two cervical immobilization techniques. The overall and first intubation attempt success rate, time to intubation, dental compression and airway visualization according to the Cormack and Lehane classification were assessed. Results: All physicians were able to intubate the manikin in all scenarios using the ET-View, whereas direct laryngoscopy failed in 16% with immobilized cervical spine using the patriot cervical extraction collar. The first intubation attempt success rate was higher and airway visualization was better in all three scenarios using the ET-View compared to direct laryngoscopy. Conclusion:The ET-View offered much better 62 airway visualization and provided higher overall and first intubation attempt success rates. Therefore, the ET-View is a valuable alternative in patients with difficult intubation due to immobilized cervical spine. Further clinical trials are indicated to confirm these findings.

INTRODUCTION:The ability to perform endotracheal intubation is one of the basic skills that paramedics should demonstrate. However, in some cases where lung or bronchial injuries have occurred, it may be necessary to separate the lung by attaching a bronchial blocker and ventilating single lung.The aim of the study was to evaluate the effectiveness of bronchial protection with VivaSight-EB bronchial blocker tube and one-lung ventilation by paramedics in conditions of simulated lung injury. MATERIAL AND METHODS:The study was a prospective randomized cross-over simulation trial in which 27 paramedics took part. In the study, paramedics performed endotracheal intubation with ETView SL and then introduced a VivaSight-EB bronchial blocker and one-lung ventilation. The bronchial protection procedure took place in two scenarios: (A) normal airway; (B) difficult airway. VivaSight-EB bronchial blocker introduction time (T1) and one-lung ventilation time (T2) and the efficacy of the procedure were analysed. RESULTS:The efficacy of endotracheal intubation with ETView SL tube was 100% during both Scenario A and Scenario B. The efficacy of endotracheal intubation with ETView SL tube was 100%. The effectiveness of the first attempt to protect the bronchi with a bronchial blocker was 26% for both scenarios and the total effectiveness of bronchial protection was 100% in both cases. Time T1 for endotracheal intubation was 18sec for Scenario A (IQR; and 20 sec for Scenario B (IQR;[17][18][19][20][21][22][23][24][25]. The time to perform one-lung ventilation (Time T2) in scenarios A and B was 43 sec (IQR; 38-62.5) vs. 43.5 sec (38-65). CONCLUSIONS:The simulation study confirmed the research hypothesis that the paramedics were able to perform one-lung ventilation using the ETView-Blocker® bronchial blocker tube after a short training.

Background: Tracheoscopic ventilation tube (TVT) is a specially designed single-lumen endotracheal tube with a camera. It was developed to facilitate endobronchial blocker insertion without bronchoscopy; its ability to explore anatomy received attention for difficult intubations. To clarify the feasibility of TVT in difficult intubation, we evaluated the learning curves of intubation between novice and expert.Methods: 182 patients who presented as Cormack-Lehane (CL) grade IIb and III with cervical in-line stabilization, and 4 trainees (2 novices, 2 experts) at single tertiary care teaching university hospital.All trainees performed intubation with TVT during laryngoscopy. Intubation attempts were limited to two times, each within 30 seconds. For every attempt, trainees visualized an imaginary pathway from the teeth to vocal cords and then shaped the stylet. Intubation was confirmed by three successive ETCO 2 measurements > 30 mmHg. Using CUSUM analysis, the trial was continued until every trainee reached an acceptable failure rate.Results: Patients were constituted with 94.5% CL grade IIb and 5.5% grade III. The median number of acceptable performances (10% of the acceptable failure rate) was 36. Overall failure rate was 5.5% (95%CI: 2.2-8.8%), with 6.9% (95%CI: 2.0-11.8%) for novices and 3.7% (95%CI: 0.0-7.8%, P=0.165, Cohen's h=0.14) for experts. Intubation time was longer in novices by about 3 seconds compared to experts (mean difference=2.8, 95%CI: 1.3-4.3, P<0.001, Cohen's d=0.57).Conclusions: Intubation with TVT in CL grades IIb and III was easy to learn and could be an alternative for difficult intubation. It required small cases to reach acceptable performance, and provided a short learning period even for novice anesthesiologists, with failure rates similar to those of experienced anesthesiologists.
