Can Patients with Common Variable Immunodeficiency Have Allergic Rhinitis?

Abstract: In CVID patients, chronic rhinitis may be allergic, because many have personal and family histories suggestive of atopy. However, in this study, allergy was confirmed by specific IgE detection in only 5.6% of cases. CVID patients with a history suggestive of AR commonly present negative results on traditional testing, so additional experiments may be necessary. One suggestion for the investigation of AR in CVID patients would be nasal provocation with the most prevalent allergens.

“…The other study where PID patients were evaluated recruited only common-variable immunodeficiency (CVID), and verified the presence of allergen sensitization in approximately 5.6% of the cases [14]. Allergen sensitization was observed in only 3.28% of the patients with CVID and an asthma diagnosis.…”

“…Allergic diseases and allergen sensitization in a group of patients with PID have previously only been reported in 2 studies. One of these, performed in Turkey, presented a particular situation where 32.39% of the cohort of PID patients had a diagnosis of IgA deficiency or IgG-subclass deficiency, both conditions that usually present fewer infections than other PID [13,14]. This is a different profile of PID patients from that of our cohort, probably because our immunology clinic receives patients referred for recurrent infections, meaning the number of patients with an IgA or IgG-subclass deficiency is very low.…”

“…100 Likewise, the same group found allergic rhinitis to be equally unusual. 101 Although 82% of the patients had a history of chronic or episodic rhinitis or rhinosinusitis, an allergic cause was confirmed only by detection of specific IgE to aeroallergens in 5.6% of the 72 patients with CVID with a clinical history suggestive of the diagnosis. This is in contrast to patients not having CVID with low or absent IgA levels with or without IgG subclass deficiency in whom allergic diseases, particularly asthma, are more common.…”

“…Although it is not part of current diagnostic criteria for CVID [1], many patients with CVID have low or undetectable levels of IgE [2]. Rates of specific IgE (serum and/or skin testing) measurement vary from 3 to 16% [2, 3] while as many as 82% of patients with CVID report symptoms consistent with allergies [2].…”
