Can excess maternal mortality among women of foreign nationality be explained by suboptimal obstetric care?

Abstract: Objectives To test the hypothesis that the risk of postpartum maternal death in France remains significantly higher for women of foreign nationality after individual characteristics are taken into account and to examine whether the quality of care received by the women who died differs according to nationality.Design A national case-control study. “…Nonetheless two important known risk factors-age, which is a primordial factor in terms of risk of death, and nationality-could be considered for all the women. [8][9][10]16 The clinical characteristics (parity, hospitalisation during pregnancy and emergency caesarean) are not especially refined, but they can be considered as a proxy for the mother's health status, during pregnancy and, to some extent, at delivery. Residual confounding cannot be excluded, in particular educational level, income, 17 obesity, 18 or inadequacy of prenatal care.…”

“…The circumstances of these transfers are identical in countries with high rates of maternal mortality. 11 The poor quality discharge is one of the factors on which action must be taken if we want to reduce significantly the maternal death rates in poor countries. Especially since these patients arrive late and sometimes in dramatic situations.…”
