Calculation of beta dosimetry in radiation synovectomy using Monte Carlo simulation (EGS4)

Abstract: Using the EGS4 Monte Carlo code, absorbed dose rate factors were estimated for four radionuclides of interest in radiation synovectomy, an intra-articular radiation therapy to treat rheumatoid arthritis. The treatment consists of the injection of a beta-emitting radionuclide into the joint capsule in order to eliminate diseased synovium through irradiation. The radionuclides investigated are 32P, 90Y, 165Dy, and 198Au. Calculations reveal the absorbed dose factor (cGy cm2/MBq s) as a function of distance (mm) … Show more

“…227 Beta-particle dosimetry of various radionuclides used in radiation synovectomy, an intraarticular radiation therapy to treat rheumatoid arthritis was also estimated using the EGS4 Monte Carlo code. 228,229 It seems as if we are on the way to a more and more personalized human dosimetry with radiolabeled antibody dosimetry one of the aims. The dose distribution pattern is often calculated by generalizing a point source dose distribution, 230,231 but a direct calculation by Monte Carlo techniques is also frequently reported because it allows media of inhomogeneous density to be considered.…”

“…The absorbed fractions for different radioisotopes and target/source pairs normally are calculated from computer-based Monte Carlo methods, which use probabilistic models to track the interactions and fate of photons or electrons from the time they are emitted from the radioisotope. 23,24 For most situations it is not necessary to calculate S values since the S values for many different sourcetarget organ pairs and radioisotopes are published in the Medical Internal Radiation Dosimetry (MIRD) tables. 25 The S values listed in the MIRD tables have been calculated assuming average physiological conditions and organ sizes.…”

“…Unter Berücksichti-gung verschiedener, unabhängig erstellter Berechnungen und Näherungen kann als beispielgebende Referenz für eine angenommene Aktivität von 220 MBq 90 Y im Kniegelenk eine an der Synovialisoberfläche wirksame Strahlendosis im Bereich von etwa 40-95 Gy angegeben werden (4,9,22,23,37).…”
