C3, factor B, alpha-1-antitrypsin in neonatal septicaemia with sclerema.

Abstract: SUMMARY C3, factor B, and x-l-antitrypsin were determined in newborn infants with septicaemia and sclerema, associated with suspected infections, ABO or Rh incompatibility, and hyperbilirubinaemia of unknown origin, during and after treatment with exchange transfusion. Activation products from C3 and factor B, the clearance of the transfused C3, and its synthesis by the recipient were determined also. Infected newborn infants had low levels of C3 and factor B, but a normal amount of x-1-antitrypsin. Exchange t… Show more

“…In 1980, as part of a larger study on septicemia in the neonatal period, 11 newborns were described with concomitant sclerema (36). Besides an opsonin defect, low levels of C3 and factor B were noted, which were felt to contribute to the pathogenesis of the sepsis and possibly the sclerema.…”

“…Induration commonly appears first over the lower extremities, especially the calves, and extends cephalad to involve almost the entire skin with the exception of the palms, soles, and scrotum (4, 17,25,26,[38][39][40]. Septicemia is a frequent and often fatal complication (36).…”

“…Using a quantitative assay Pelet et al (8) found only 2 out of 11 newborns with sepsis to have elevated plasma C3d concentrations. Even infants with a fatal outcome of sepsis had no increase of C3d.…”

“…In eight newborns (Median: 35th week of gestational age) with generalized bacterial infection we did not find any baby with increased C3d (Nurnberger & Wahn, unpublished results). Perhaps the defective Factor B activation (8) and/or the defective activation of the alternative pathway in newborns (9) contributes to this failure of C3d generation in newborns.…”
