C-Reactive Protein Concentration in Canine Idiopathic Polyarthritis

Abstract: ABSTRACT. To investigate the clinical utility of C-reactive protein (CRP) in idiopathic polyarthritis (IPA), its concentration was measured in dogs with IPA. The CRP concentration was markedly increased in all the IPA dogs at the time of diagnosis and decreased significantly in response to the initial corticosteroid treatment; this indicated that CRP can be used as an index for therapeutic response in IPA cases. Furthermore, at 6 months after the diagnosis, a significant association was observed between the CR… Show more

“…Recent investigations into canine CRP levels have reported elevated levels in a number of disease states including irritable bowel syndrome (IBD) (Jergens et al 2003) and autoimmune haemolytic anaemia (AIHA) (Tecles et al 2005). A marked increase in CRP levels was noted in all of the canine immunemediated arthritis cases analysed in a recent study (Ohno et al 2006). A high level was also observed in this study with an average of 6.9 ± 6.7%.…”

“…In human medicine, CRP has been reported to indicate the severity and prognosis of various diseases [9,10,16,17]. In veterinary medicine, it has been reported to indicate the response to treatment and prognosis in idiopathic polyarthritis [14]. Further studies are needed to examine the variation in the CRP concentration during clinical courses and to evaluate its relationship with disease severity in order to determine its potential as a prognostic factor.…”

“…In Japan, a laser nephelometric immunoassay (LNIA), for canine CRP, was developed in 1998 [13,14] and is now commercially available. In this assay, CRP and anti-canine CRP antiserum are mixed in a specific buffer, exposed to a laser beam, and the scattered light is then measured.…”

“…In the current study, although serum CRP concentration was elevated on presentation in 21 out of 32 dogs, a single CRP determination did not facilitate prediction of outcome, a finding that has been previously reported in dogs with various inflammatory conditions. 3,7,8,12,13,21 Only 3 out of the 28 dogs that had serial CRP concentrations died. Figure 1 suggests that average initial CRP concentrations are predicative of survival.…”

“…Although CRP response can vary dramatically in different individuals with the same disease process at presentation, evaluating sequential measurements and degree of decrease in serum CRP concentrations may provide more useful information than a single CRP value. Generally, serum CRP concentrations in dogs with inflammatory bowel disease, 10 immune-mediated polyarthritis, 11,21 and immune-mediated hemolytic anemia 7,16 decrease as clinical signs and leukocytosis resolve in response to therapy. Excluding dogs that were euthanized or died soon after initial CRP analysis in the current study, 88% of dogs with elevated CRP values at presentation had decreases in CRP concentrations by 48-72 hr.…”
