Досліджували вміст інтерлейкінів (ІЛ) ІЛ-1β, ІЛ-6, ІЛ-8, фактора некрозу пухлин α (ФНП-α) ВСТУПУ клінічній практиці актуальною залишаєть-ся проблема своєчасної діагностики і такти-ки ведення хворих з гострим абдомінальним болем, оскільки симптом "гострого живота" супроводжує велику кількість соматичних захворювань. До групи захворювань, які викликають гострий абдомінальний біль, входять вогнищеві гнійні запальні процеси органів черевної порожнини (ОЧП). Важ-ливою є диференційна діагностика абдо-мінальних захворювань хронічної природи (абдомінальний туберкульоз -АТ) та гостро-го неспецифічного мезентеріального лімфа-деніту, які можна лікувати консервативно, з деструктивними формами апендициту, що потребують хірургічного втручання. АТ клі-нічно може перебігати "під маскою" гострого апендициту, тому його складно діагностувати і лікувати [1][2][3][4][5]. Основним патофізіологічним механізмом формування абдомінального бо лю є запалення та ішемія. В контексті сучасної концепції про системне запалення [6] значення імунологічних досліджень для хворих хірургічного профілю з гострою запальною патологією значно зростає. Як відомо, цитокіни регулюють проліферацію та диференціацію імунокомпетентних клітин, міграцію у вогнище запалення, їх активацію і перетворення в ефекторні клітини. Нормальне функціонування імунної системи забезпечу-ється балансом продукції і акцепції про-та протизапальних цитокінів, які координують імунну відповідь та інтенсивність запальної реакції [7][8][9][10][11]. Дисбаланс у системі цитокінів може порушити ці взаємозв'язки, сприяти генералізації запалення та інфекції і стати од-нією з провідних причин розвитку синдрому системної запальної відповіді та летальності [6,9,11]. Покращення диференційної діагностики запальних захворювань ОЧП дасть можливість зменшити кількість негативних апендектомій, які створюють непотрібні ризики та затрати коштів як для пацієнта, так і для лікувального закладу [15,16]. Infections remain an important cause of morbidity and mortality early after liver transplantation. The aim of this prospective longitudinal study was to evaluate clinical utility of c-reactive protein (CRP), procalcitonin, and neutrophil-to-lymphocyte ratio (NLR) in surveillance of infections early after liver transplantation in intensive care setting. A total of 60 liver transplant recipients were included. CRP, procalcitonin, and NLR assessed at 12-hour intervals were primary variables of interest. Infections and severe complications during postoperative intensive care unit stay were the primary and secondary end-points, respectively. Infections and severe complications were diagnosed in 9 and 17 patients, respectively. Only peak CRP beyond first 48 hours was associated with infections (p = 0.038) with AUC, positive and negative predictive value of 0.728, 42.9% and 92.2%, respectively (cut-off: 142.7 mg/L). Peak procalcitonin over first 60 hours was the earliest predictor (p = 0.050) of severe complications with AUC, positive and negative predictive value of 0.640, 53.3% and 80.0%, respectively (cut-off: 42.8 ng/mL). In conclusion, while CRP, procalcitonin, and NLR cannot be used for accurate diagnosis of infections immediately after liver transplantation, peak CRP beyond 48 hours and peak procalcitonin over first 60 hours may be used for initial exclusion of infections and prediction of severe complications, respectively.Despite remarkable improvement of outcomes of patients after liver transplantation over past decades, infections remain one of the most important threats and most common causes of death in a population of liver transplant recipients 1-3 . In the early post-transplant period, they are the most common cause of morbidity 4 . Even 70% of liver transplant recipients develop bacterial infections, with vast majority occurring during the first post-transplant month 5,6 . These complications remain an important cause of death in the early post-transplant period with overall infection-related mortality rates varying between 0-30% depending on etiology, timing, and site 7-10 . Particularly high mortality rates, in the range of approximately 35-70%, are reported for patients developing infections with multiple drug resistant microorganisms, such as extended spectrum beta-lactamase-producing or carbapenem-resistant Enterobacteriaceae 11-13 . Improvements in prevention, early diagnosis and management of infections in the early period after liver transplantation are thus undoubtedly necessary to further improve patient outcomes. Objective The development of these updated clinical practice guidelines (CPGs) was commissioned by the American Association of Clinical Endocrinologists (AACE), The Obesity Society (TOS), American Society for Metabolic and Bariatric Surgery (ASMBS), Obesity Medicine Association (OMA), and American Society of Anesthesiologists (ASA) Boards of Directors in adherence with the AACE 2017 protocol for standardized production of CPGs, algorithms, and checklists. Methods Each recommendation was evaluated and updated based on new evidence from 2013 to the present and subjective factors provided by experts. Results New or updated topics in this CPG include: contextualization in an adiposity‐based chronic disease complications‐centric model, nuance‐based and algorithm/checklist‐assisted clinical decision‐making about procedure selection, novel bariatric procedures, enhanced recovery after bariatric surgery protocols, and logistical concerns (including cost factors) in the current health care arena. There are 85 numbered recommendations that have updated supporting evidence, of which 61 are revised and 12 are new. Noting that there can be multiple recommendation statements within a single numbered recommendation, there are 31 (13%) Grade A, 42 (17%) Grade B, 72 (29%) Grade C, and 101 (41%) Grade D recommendations. There are 858 citations, of which 81 (9.4%) are evidence level (EL) 1 (highest), 562 (65.5%) are EL 2, 72 (8.4%) are EL 3, and 143 (16.7%) are EL 4 (lowest). Conclusions Bariatric procedures remain a safe and effective intervention for higher‐risk patients with obesity. Clinical decision‐making should be evidence based within the context of a chronic disease. A team approach to perioperative care is mandatory, with special attention to nutritional and metabolic issues.
