Una de las áreas de acción que constantemente está presentando nuevos retos en fonoaudiología es el Trastorno Cognitivo Comunicativo (TCC). Esto se debe, en parte, a la heterogeneidad del cuadro clínico. Debido a dicha heterogeneidad, surge la necesidad de contar con subclasificaciones que den cuenta de las características distintivas de los distintos perfiles que pueden ser observados en este trastorno. Dado lo anterior, el objetivo de este trabajo fue determinar si era posible agrupar las características observadas en el TCC para crear subclasificaciones. Para ello se realizó una revisión sistemática de artículos que abordan la temática. Para efectuar la búsqueda se consultaron las bases de datos “PubMed” y “Web of Science” (WoS) incluyendo los términos (cognitive-communication disorder OR cognitive-communication impairment). Las características cognitivo-comunicativas identificadas en cada uno de los artículos se analizaron de modo categorial, permitiendo generar potenciales agrupaciones de acuerdo con el grado de convergencia que presenten los hallazgos con macro categorías a las cuales puedan subsumirse. Los resultados muestran que existen tres perfiles asociados a este trastorno, uno asociado a dificultades en las habilidades cognitivas basales, otro a dificultades en las habilidades comunicativas pragmáticas y un tercero que presenta dificultades en ambas áreas. Se concluye que la subclasificación de TCC es viable dada la convergencia de las dificultades evidenciada. Éste se puede clasificar en TCC Ejecutivo (TCCe), TCC Pragmático (TCCp) y TCC Ejecutivo-Pragmático (TCCep).

Burn injury is a devastating disease with high incidence of disability and mortality. The cognitive dysfunctions, such as memory defect, are the main neurological sequelae influencing the life quality of burn-injured patients. The post-burn cognitive dysfunctions are related to the primary peripheral factors and the secondary cerebral inflammation, resulting in the destruction of blood-brain barrier (BBB), as is shown on Computed Tomography (CT) and magnetic resonance imaging examinations. As part of the neurovascular unit, BBB is vital to the nutrition and homeostasis of the central nervous system (CNS) and undergoes myriad alterations after burn injury, causing post-burn cognitive defects. The diagnosis and treatment of cognitive dysfunctions as burn injury sequelae are of great importance. In this review, we address the major manifestations and interventions of post-burn cognitive defects, as well as the mechanisms involved in memory defect, including neuroinflammation, destruction of BBB, and hormone imbalance.

To examine changes in functional memory, problem solving, comprehension, expression, and social communication over the first 2 years posttraumatic brain injury (TBI) and the ability of each to predict return to work (RTW) outcomes at 1 year and 5 years postinjury. Design: Secondary analysis of data from a multicenter longitudinal cohort study. Setting: Acute inpatient rehabilitation facilities and community follow-up. Participants: A total of 3543 individuals between 16 and 60 years of age who were competitively employed at the time of TBI and had completed year 1, year 2, and year 5 postinjury follow-ups. Main Outcome Measures: Year 1 and year 5 RTW status (± competitively employed) at the time of study completion. Results: Greater function across each of the 5 cognitive-communication abilities was associated with RTW success at 1 year and 5 years post-TBI. At discharge, these 5 abilities showed comparable odds of predicting later employment. At year 1 and year 2 followups, independence with problem solving was the most predictive of employment 5 years post-TBI, followed by social interaction, memory, expression, and comprehension. Conclusions: An increased rehabilitation focus on functional memory, problem solving, comprehension, expression, and social interaction post-TBI has the potential to improve RTW outcomes.
