Introduction: Infants represent an important risk group for influenza associated hospitalizations and mortality. This study evaluated the clinical presentations, hospitalization course and outcome of infants hospitalized with the pandemic influenza A H1N1 [Influenza A(H1N1)pdm09] in relation to their previous health status. Methodology: We conducted a retrospective chart review of hospitalized infants with laboratory-confirmed Influenza A(H1N1)pdm09 infection in two hospitals in Kuwait. Demographic characteristics, pre-existing high-risk medical conditions, clinical presentations, complications and mortality were analyzed. Previously healthy infants' data were compared with infants with pre-existing high-risk medical conditions for severity of the illness and outcome. Results: We identified 62 infants comprising 32% of all admissions with Influenza A(H1N1)pdm09. The median age  SD was 7  4 months. Nineteen (31%) had pre-existing high-risk medical conditions. Complications were documented in 53% of previously healthy infants compared to 47% in high-risk infants. Mean duration of hospitalization was 4.9 days in healthy infants and 6.7 for infants with highrisk medical conditions. Bacterial pneumonia complicated 7% of previously healthy infants compared to 26% with high-risk conditions (P = 0.03). Four infants (6.5%) required admission to the intensive care unit (ICU), of whom three had high risk medical condition. Conclusion: The majority of hospitalized infants with Influenza A(H1N1)pdm09 were previously healthy. Prolonged hospitalization, ICU admission and mortality were more observed in infants with high-risk medical conditions. According to the latest Advisory Committee on Immunization Practices (ACIP) recommendations, annual influenza vaccination is recommended for any child six months of age and older, particularly those with risk factors.

BackgroundInfluenza is a vaccine preventable disease that causes important morbidity and mortality worldwide. Estimating the burden of influenza disease is difficult. However, there are some methods based in surveillance data and laboratory testing that can be used for this purpose.ObjectivesEstimating the burden of serious illness from influenza by means of hospitalization and death records during the period between 2012 and 2014, and using information from Severe Acute Respiratory Illness (SARI) surveillance.MethodsTo estimate the Chilean rate of influenza‐associated hospitalizations and deaths, we applied the influenza positivity of respiratory samples tested in six SARI surveillance sentinel hospitals to the hospitalizations and deaths from the records with ICD‐10 codes from influenza and pneumonia.ResultsAnnually, 5320 people are hospitalized for influenza and 447 die for this cause. The annual influenza‐associated hospitalization rate for the period was 71.5/100 000 person‐year for <5 years old, 11.8/100 000 person‐year for people between 5 and 64 years old; and 156.0/100 000 person‐year for ≥65 years. The annual mortality rate for the period was 0.08/100 000 person‐year for <5 years; 0.3/100 000 person‐year for people between 5 and 64 years; and 22.8/100 000 person‐year for ≥65 years.ConclusionsThis is the first study of influenza burden in Chile. Every year an important quantity of hospitalizations and deaths result from influenza infection. In countries in temperate zones, it is important to know the burden of influenza in order to prepare the health care network and to assess preventive intervention currently in practice and the new ones to implementing.

In this study the patients diagnosed as influenza infection during the year 2014 were investigated, and evaluation of the differences between patients followed up on an outpatient basis, and those requiring hospitalization. Material and Method: Forty-eight patients diagnosed as influenza based on PCR-positive nasopharyngeal smears obtained from patients followed up as an inpatient or on an ambulatory basis according to their clinical status and hospitalized patients in Istanbul University, Faculty of Medicine, Department of Children's Health and Diseases between January2014-December 2014 were included in the study.Patients' medical records and laboratory analysis were retrospectively analyzed. [n=22, 45.8%, median 43.5 months, (4-204 months)] was statistically smaller than the median age of ambulatory patients [n=26, 54.2%, median 92.5 months, (4-204 months)] (p=0.002). Fever was the most common presenting symptom (n=38, 79,2%). Twenty-five patients (52.1%) had chronic diseases with the most common one being asthma (n=5, 10,4%). Only one patient (2.1%) had influenza vaccination. Influenza types were H1N1 (43,8%), H3N2 (27,1%) and Influenza B (n=14, 29,2%). Neutropenia was the most common pathologic laboratory finding (n=8, 16,7%). Median hemoglobin value of the hospitalized cases [median 11,7 gr/dL (7,4-1,3 gr/dL)] was significantly lower than that of the patients followed up on an ambulatory basis [median 12,4 gr/dL, (9,9-14,6 gr/ dL)] (p=0,02). In total, median duration of the disease was 10 days (7-21 days). A patient (2,1%) with the diagnosis of chronic liver disease died secondary to acute respiratory distress syndrome. Results: Median age of the patients (female, n=16, 33.3%) was 54 months (4-204 months). The median age of hospitalized patients Conclusion:In order to decrease complications and hospitalization rates related to influenza, small children and the patients with chronic disorders should be vaccinated.
