There were 727 million persons aged 65 years or over in the world in 2020. Over the next three decades, the global number of older persons is projected to more than double, reaching over 1.5 billion in 2050 The aim of this study was to assess burden and coping mechanism among care giver for old age with advanced illness. A descriptive design is utilized in this study. A Convenience sample of caregivers was used it composed of 100 of caregivers and their patients who match the criteria: primary caregivers who provide care for the patients for at least one year and accept to participate in the study. Two tools were utilized 1: Interviewing questionnaire composed of 3 parts, Socio-demographic data, medical history of the patients, coping strategies used by the care giver to reduce 2 to assess burden of caregivers. The result the current study revealed (43%) of the studied caregivers reported high level of burden and stress, flowed (32%) reported medium level of burden and (25%) reported low level of burden. This study conducted that caregiver factors as Age, gender, educational level, income status, knowledge of the diseases, and coping., patient factors (Age, clinical symptom, and disability in daily life), and environment factors (Health services and its utilization, and social support.) are major factors that affected caregivers' burden. The present study recommended that Provide guideline as well as clinical training to assist caregivers of chronic disease patients at home This helps them deal with the burden of care and manage the stress associated with it.

Background: Today, people are living longer than ever before due to advances in education, technology, medicine, food distribution, and public health. Longevity has also resulted in a caregiving burden in the family living together. The purpose of this paper is to analyze the situation of the elderly caregiving burden in Nepal. Methods: This is a cross-sectional study carried out in a rural area of Nepal. The total sample size for this study was 150 older persons 65 years and above. The caregiving burden was measured through the widely used scale Zarit Burden Interview (ZBI). Results: Mean age of the care recipients (elderly) was 78.46 (±7.78) years and the mean age of the care provider was 46.6 (±46) years. The mean score of the caregiving burden was 12.89 (±5.7). The majority of the caregiver has reported little or no burden (88%), 10% mild to the moderate burden, 1.33% moderate to severe burden and only one respondent (0.67%) has reported severe burden. Marital status, ethnicity, religion, living arrangement, and functional limitations on activities on daily living were significant variables related to caregiving burden. Conclusion: This study shows the caregiving burden is quite low among Nepalese care providers living in a rural area. Given the growing number of elderly people, there is an urgent need to care for the wellbeing of dependent older people and their families.

Extent and Predictors of Decision Regret among Informal Caregivers Making Decisions for a Loved One: A Systematic Review

Background Informal caregivers often serve as decision makers for dependent or vulnerable individuals facing health care decisions. Decision regret is one of the most prevalent outcomes reported by informal caregivers who have made such decisions. Objective To examine levels of decision regret and its predictors among informal caregivers who have made health-related decisions for a loved one. Data sources We performed a systematic search of Embase, MEDLINE, Web of Science, and Google Scholar up to November 2018. Participants were informal caregivers, and the outcome was decision regret as measured using the Decision Regret Scale (DRS). Review Methods Two reviewers independently selected eligible studies, extracted data, and assessed the methodological quality of studies using the Mixed Methods Appraisal Tool. We performed a narrative synthesis and presented predictors of decision regret using a conceptual framework, dividing the predictors into decision antecedents, decision-making process, and decision outcomes. Results We included 16 of 3003 studies identified. Most studies ( n = 13) reported a mean DRS score ranging from 7.0 to 32.3 out of 100 (median = 14.3). The methodological quality of studies was acceptable. We organized predictors and their estimated effects (β) or odds ratio (OR) with 95% confidence interval (CI) as follows: decision antecedents (e.g., caregivers’ desire to avoid the decision, OR 2.07, 95% CI [1.04–4.12], P = 0.04), decision-making process (e.g., caregivers’ perception of effective decision making, β = 0.49 [0.05, 0.93], P < 0.01), and decision outcomes (e.g., incontinence, OR = 4.4 [1.1, 18.1], P < 0.001). Conclusions This review shows that informal caregivers’ level of decision regret is generally low but is high for some decisions. We also identified predictors of regret during different stages of the decision-making process. These findings may guide future research on improving caregivers’ experiences.
