Midinfrared Laser Pancorneal Coagulation as a Method of Treatment for Painful Bullous Keratopathy

“…These procedures are based on the thermomechanical formation of defects of the Bowman membrane, which, on the one hand, results in the cicatrization and tight attachment of the epithelial layer, thus decreasing the possibility of the detachment of the epithelium from the Bowman membrane, the basic cause of the development of bullae and erosions; however, on the other hand, it does not resolve the edema of deep corneal layers. 11,14 Unlike ASP, cautery, ALK, and PTK that use either thermomechanical effects (in the case of ASP or cautery) or direct tissue-removing effects involving only superficial corneal stromal layer, PCC uses multiple 3-dimensional coagulation lesions involving both superficial and deep corneal stromal layers. 18 For the resolution of the pain syndrome in bullous keratopathy, excimer laser PTK, 19 including that combined with other treatment modalities, particularly, AMT, may be also used; however, any use of AMT requires donor material.…”
