Bullae-Forming Pulmonary Metastasis from Choriocarcinoma Presenting as Pneumothorax

Abstract: Gestational trophoblastic disease (GTD) is a condition of uncertain etiology, choriocarcioma, or placental-site hydatidiform moles, invasive moles, choriocarcinoma, and placental-site trophoblastic tumors. It arises from the abnormal proliferation of trophoblastic tissue and spreads beyond the uterus hematogenously. The early diagnosis of GTD is important to ensure timely and successful management and the preservation of fertility. We report the unusual case of a metastatic choriocarcinoma that formed bullae o… Show more

“…Other patterns of pulmonary metastatic disease have been reported and can be detected with chest CT. A common pattern is a small nodule sur-rounded by a halo of ground-glass attenuation, the so-called halo sign, which results from peritumoral hemorrhage (Fig 17) (59). Cavitary and bulla-forming pulmonary metastases may occur, which can result in pneumothorax (60,61).…”

“…In the literature, a handful of reports of these rare pulmonary complications have been described. A case of bullae-forming choriocarcinoma with PTX formation was reported in South Korea ( Hyun et al, 2015 ). A case of intermediate trophoblastic disease presenting as PTX was reported in 2015 ( Multani et al, 2015 ).…”

“…On CT a common pattern is a peripheral ground-glass attenuation surrounding the nodules (halo sign), which results from peritumoral hemorrhage [14] . In some cases, cavitary and bulla-forming pulmonary metastases may occur, which can result in pneumothorax [15] . Brain metastases are usually multiple, located at grey-white matter junctions, with hemorrhage and surrounding edema.…”
